At a glance
- Drospirenone is a progestin with antimineralocorticoid and antiandrogenic effects, which sets it apart from older progestins.
- A pill combining drospirenone with low-dose ethinyl estradiol is approved in the US to treat PMDD symptoms in women choosing it for contraception.
- A 2023 Cochrane review of five randomized trials (858 women analysed, mostly with PMDD) found some benefit over placebo, with evidence quality ranging from very low to moderate.
- In one trial, 48% of women on drospirenone saw a 50% drop in symptom scores, versus 36% on placebo.
- It's a treatment a doctor prescribes and monitors over several cycles, not something to reach for in the middle of a hard moment.
What is drospirenone?
Drospirenone is a synthetic progestin, a lab-made hormone that acts like progesterone in the body. Unlike older progestins, it has extra effects, covered in a 2007 drug profile published in Drugs:
- Acts like progesterone at progesterone receptors
- Blocks the receptor aldosterone acts on, an antimineralocorticoid effect
- Blocks androgen receptors, an antiandrogenic effect
It's combined with a low dose of ethinyl estradiol, a form of estrogen, in certain combined oral contraceptive pills. Many of these pills use a 24-day active, 4-day inactive schedule, so you take hormones most of the month instead of the usual three weeks on, one week off.
That schedule keeps estrogen and progesterone steadier across the cycle than the natural rise and fall described on the menstrual cycle hormones page. A steadier hormonal pattern is why researchers started testing drospirenone for premenstrual symptoms, not just contraception.
How does it relate to PMS and PMDD?
PMDD is a severe, diagnosable form of PMS: at least five symptoms, including a mood symptom, confirmed by daily tracking across two cycles. PMS covers milder physical or emotional symptoms in the one to two weeks before a period.
In the US, a drospirenone-containing pill is approved specifically for treating PMDD symptoms in women who are already choosing a combined pill for contraception, not as a standalone PMDD treatment on its own.
The reasoning follows from what causes PMDD: PMDD comes from how some brains react to the normal rise and fall of estrogen and progesterone, not from abnormal hormone levels.
Suppressing that natural rise and fall with a steady, low dose of hormones can ease symptoms for some women, though it works differently for each person.
Drospirenone pills sit alongside other options a doctor weighs together, not as a first or only choice:
- SSRIs, including some taken only in the luteal phase
- Other hormonal contraceptives
- CBT
What does the research say about drospirenone and PMDD?
Several randomized trials have tested drospirenone-containing pills against placebo for PMDD symptoms, measured with daily symptom diaries.
| Study | Women studied | What it found |
|---|---|---|
| Cochrane review, 2008 | 1,600 (5 trials) | Two placebo-controlled PMDD trials showed less severe symptoms after three cycles (mean difference -7.83 points) |
| RCT, Obstetrics & Gynecology, 2005 | 450 | 48% had a 50% drop in symptom scores, versus 36% on placebo |
| Crossover RCT, Contraception, 2005 | 64 | 61.7% responded on drospirenone, versus 31.8% on placebo |
| Cochrane review, 2023 | 858 (5 trials, mostly PMDD) | Some benefit over placebo, but evidence quality ranged from very low to moderate |
A cycle-by-cycle subanalysis found the biggest symptom drop happened in the first treatment cycle, with smaller further gains through cycles two and three. The pattern across trials is consistent: drospirenone pills beat placebo on average, but the effect is modest and the evidence isn't strong enough to call it a guaranteed fix for any one person.
What should you ask your doctor about drospirenone?
A doctor weighs this evidence against your own health history before prescribing a drospirenone pill. A few factors can rule out combined pills altogether:
- A personal or family history of blood clots
- Migraines with aura
Side effects in trials were typical of other combined pills, most likely in the first few cycles (Barnhart & Schreiber, 2007, Drugs).
None of this is something to decide alone. Niyora is a PMS and PMDD support app. If you want somewhere to process a hard moment while you wait to talk to a doctor about options like this, Niyora's Moon lets you write down what happened, see it from another side, and draft a reply you can hold onto for 20 minutes.

Does continuous paroxetine or birth control work better for PMDD?
Both antidepressants and birth control pills are effective for PMDD. Continuous paroxetine (an antidepressant) and DROS/EE 24/4 (a specific birth control) showed the strongest effects in studies reviewed, though the evidence comes from relatively few studies.

Does sleeping longer improve mood and fitness in elite athletes?
Yes. Sleeping longer was linked to better mood and fitness across all athletes, regardless of menstrual cycle status. Sleeping less than 8 hours was linked to below-average mood and fitness.

Does mood, well-being, and sexual function change across the menstrual cycle?
No, there were no meaningful changes in sexual function, mood, or well-being across menstrual cycle phases in healthy young women. Being in a stable relationship predicted better sexual function overall, but cycle phase did not.

Are irregular menstrual cycles linked to lower thinking and memory skills in middle-aged women?
Yes. Women with irregular cycles at ages 29-46 and women with very short cycles (25 days or less) at ages 18-22 both had slightly lower memory scores later in life.

Does estetrol/drospirenone birth control improve PMS symptoms?
Yes. Women who started this birth control experienced significant improvements in pain, bloating, and negative mood symptoms. Women already on other birth control saw minimal improvements.

What are the best treatments for PMDD and PMS?
SSRIs (antidepressants) and combined oral contraceptives (birth control pills) have the strongest evidence. For difficult cases, GnRH agonists and surgery work. Some alternatives like acupuncture and yoga show promise but need better quality research.

Questions people ask
Is drospirenone the same as progesterone?+
No. Drospirenone is a synthetic progestin that acts on the same receptors as progesterone, but it's chemically different and also has antimineralocorticoid and antiandrogenic effects natural progesterone doesn't have. It's combined with estrogen in certain birth control pills, not taken as a progesterone replacement on its own.
Does a drospirenone pill cure PMDD?+
No. Drospirenone pills don't cure PMDD. Cochrane reviews of randomized trials found they can ease premenstrual symptoms more than placebo for some women, but the evidence quality ranges from very low to moderate, and results vary from person to person.
How fast does drospirenone help PMDD symptoms?+
In trials, women taking a drospirenone pill for PMDD saw the biggest symptom drop in the first treatment cycle, with smaller further improvements through cycles two and three. A doctor typically gives it a few cycles before judging whether it's working.
Does drospirenone help PMS as well as PMDD?+
Most trials of drospirenone pills enrolled women with PMDD specifically, since that's the approved use. Cochrane reviews group PMS and PMDD trials together, so some evidence of benefit for milder premenstrual symptoms exists, but it's less studied than the PMDD indication.
