# PMS treatment for insomnia before your period

No single treatment has strong, direct evidence for insomnia before your period. Most research treats PMS or PMDD as a whole, with poor sleep as one symptom among several, and the one trial that tested a supplement for insomnia directly found no benefit over placebo. Treating the broader pattern, with SSRIs, CBT, or Vitex agnus-castus, is better supported than any insomnia-only remedy.

## At a glance

- No treatment has been proven specifically for premenstrual insomnia; most evidence is for PMS or PMDD overall, with sleep as one symptom among several.
- A three-cycle randomized trial testing curcumin for premenstrual insomnia found it worked no better than placebo.
- Herbal options with the most evidence, Vitex agnus-castus and chamomile, were studied for PMS symptoms broadly, not for sleep specifically.
- A systematic review of placebo-controlled trials found no reliable evidence that homeopathic remedies help premenstrual sleep problems.
- A pattern that disrupts a week of every month is worth describing to a doctor.

## Strength of evidence

- Moderate
- Strong

No treatment has been tested and found effective for premenstrual insomnia specifically. The one direct trial, testing curcumin, found no benefit over placebo. Evidence for Vitex agnus-castus and chamomile covers PMS symptoms overall, not sleep alone, and a review found no evidence that homeopathy helps.

## What's the best treatment for insomnia before your period?

No single treatment is proven specifically for insomnia before your period. Most research tests treatments for premenstrual syndrome (PMS) or premenstrual dysphoric disorder (PMDD) as a whole, with poor sleep counted as one symptom among several rather than the main outcome.

The options with the strongest track record are the same ones that help PMDD, and sometimes severe PMS: SSRIs, certain hormonal contraceptives, and cognitive behavioral therapy (CBT).

These ease the anxiety, low mood, and racing thoughts that often keep you up, even though the trials behind them weren't designed to measure sleep on its own.

See [PMS treatment: what actually helps](https://niyora.com/treatments/pms-treatment/) and [PMDD treatment: what actually helps](https://niyora.com/treatments/pmdd-treatment/) for how those work.

Among options you can buy without a prescription, Vitex agnus-castus (chasteberry) has the most consistent evidence for PMS symptoms generally, from a 2017 meta-analysis of randomized trials. Curcumin is the one supplement tested directly for premenstrual insomnia, in a 2023 trial, and it didn't outperform placebo.

| Option | What it's shown to help | Evidence strength |
| --- | --- | --- |
| SSRIs | PMDD mood and anxiety symptoms overall | Supported by PMDD treatment guidelines, not tested for insomnia alone |
| CBT | Thought patterns feeding anxiety and low mood | Supported for PMS and PMDD overall |
| Vitex agnus-castus | PMS symptoms overall | Meta-analysis of 17 randomized trials, PMS overall, not insomnia specifically |
| Curcumin | Tested directly for premenstrual insomnia | No benefit found over placebo in a 2023 trial |
| Homeopathy | Premenstrual symptoms and sleep complaints (studied separately) | No reliable evidence found in a systematic review |

## Do over-the-counter options help?

When people ask about over-the-counter options for premenstrual insomnia, they usually mean supplements rather than prescription medicine. Of those, only a few have been tested in good trials, and the results are mixed at best.

- Curcumin was tested directly for premenstrual insomnia and daytime sleepiness in a triple-masked, placebo-controlled trial. Taken as a 500 mg capsule with piperine daily from a week before the period until three days after, across three cycles, it didn't beat placebo.

- Homeopathic remedies were reviewed across placebo-controlled trials for premenstrual problems and sleep complaints. The review found no reliable evidence that they work.

- Common sleep aids like melatonin or antihistamines haven't been tested specifically for premenstrual insomnia in the research available, so there's no direct evidence either way.

None of this means nothing will help you personally. It means the research hasn't caught up to the question yet. If you try a supplement, mention it to your doctor or pharmacist first, since herbal remedies, while usually well tolerated, can still interact with other medicine.

Right now

Can't sleep right now? Try this

- 1 Get up if you've been lying awake more than 20 minutes. Dim the light and skip the phone.
- 2 Write down whatever's looping in your head, even messy and half-finished, so it's not just circling.
- 3 Breathe out for six counts, in for four, and repeat until your shoulders drop a little.
- 4 If a fight or a worry is what's keeping you up, Niyora's Moon can help you write it out and put it down before you try again.

Niyora walks you through this when it happens:

- Calm down

## What natural options have evidence?

Most natural options studied for premenstrual symptoms were tested for PMS overall, not sleep specifically. Here's what the evidence says about the main ones.

- Vitex agnus-castus (chasteberry): the most-studied herbal option for PMS. A 2017 meta-analysis of randomized trials found it eased PMS symptoms more than placebo. A separate meta-analysis on breast pain used doses of 20 to 40 mg a day for three months, though that trial measured pain, not sleep.

- Chamomile: a 2019 systematic review of eight randomized trials found it eased PMS symptoms overall, likely through calming, anti-anxiety plant compounds, though none of the trials measured insomnia on its own.

- Other herbs in regional trials: a review of Iranian herbal medicine research lists saffron, lemon balm (Melissa officinalis), and valerian root among herbs used for PMS, though insomnia wasn't the specific measure in those trials either.

- Curcumin: the one herbal option tested directly against premenstrual insomnia, and it didn't work better than placebo over three cycles.

**None of these are risk-free.** A broader review of herbal remedy safety found they're generally better tolerated than prescription drugs, but they can still cause side effects or interact with other medicine. Tell your doctor about any supplement before you start it.

## When should you start taking something?

Timing follows the trial you're copying. The curcumin study that tested insomnia directly started dosing seven days before the period, continued three days into it, and repeated this for three menstrual cycles before measuring results. That's one concrete pattern for luteal-phase supplement timing, even though the result itself was negative.

For prescription options, SSRIs for PMDD can be taken every day of the cycle or only in the luteal phase, the two weeks before your period, depending on what your doctor recommends based on your pattern. CBT and other overall PMS or PMDD treatments usually run on their own schedule, not tied to a specific cycle day.

- Track your symptoms daily for one to two cycles first, so you know your actual pattern rather than guessing. The [DRSP chart](https://niyora.com/tools/drsp-chart/) is built for this.

- If you try an herbal option, give it the same three-cycle window used in trials before deciding whether it's working.

- If you're considering a prescription option, bring your tracked pattern to your doctor so they can decide whether a luteal-phase-only schedule fits you.

While you wait to see what helps, having somewhere to put the thoughts that keep circling at night can make the waiting easier. Niyora is a PMS and PMDD support app. Its Moon tool lets you write down what's looping, see it from another angle, and calm down before you try to sleep again.

## When does insomnia before your period need medical help?

If poor sleep before your period is part of a pattern severe enough to disrupt your week, every month, it's worth more than a supplement.

A doctor can also rule out other causes of insomnia, premenstrual or not, like thyroid issues, anxiety disorders, or sleep apnea, before settling on PMS or PMDD as the explanation. If you're not sure where to start, [finding a PMDD specialist](https://niyora.com/care/pmdd-specialist/) covers how to find someone who treats this specifically.

When to see a doctor

If insomnia and other premenstrual symptoms take up a week of every month, or you're having thoughts of hurting yourself, see a GP or gynecologist. Bring two months of daily notes.

## Questions people ask

Do over-the-counter sleep aids help with PMS insomnia?

+

Few over-the-counter options have been tested directly for premenstrual insomnia. A triple-masked trial found curcumin didn't outperform placebo for insomnia or daytime sleepiness. A separate review found no reliable evidence that homeopathic remedies help either. Common aids like melatonin haven't been studied specifically for premenstrual insomnia, so there's no evidence yet either way.

What natural remedy has the best evidence for PMS symptoms?

+

Vitex agnus-castus (chasteberry) has the most consistent evidence among herbal options for PMS overall, based on a meta-analysis of randomized trials. Chamomile also showed benefit for PMS symptoms in a systematic review of eight trials. Neither was tested for insomnia specifically, so their effect on sleep alone isn't proven.

When should I start taking something for premenstrual insomnia?

+

Track your symptoms daily for one to two cycles first, so you know your actual pattern. If trying a supplement, follow the timing used in trials, roughly a week before your period through a few days into it, for at least three cycles before judging results. A doctor can advise on prescription timing.

Can SSRIs help with premenstrual insomnia?

+

SSRIs are a well-supported treatment for PMDD overall, easing mood and anxiety symptoms that often disrupt sleep, though trials don't isolate insomnia as the outcome. They can be taken daily or only in the two weeks before your period, depending on what your doctor recommends for your pattern.

When does premenstrual insomnia need medical attention?

+

Premenstrual insomnia needs medical attention when it disrupts a week of every month, or if you have thoughts of hurting yourself. A doctor can rule out causes like thyroid issues or sleep apnea, and two months of daily notes make the pattern easier to show them.
