# Treatment for PMS depression before your period

SSRIs are the best-studied treatment for premenstrual depression, and they work even when taken only in the luteal phase. Hormonal contraceptives and CBT help some people too, while herbal options and light therapy have weaker, inconsistent evidence. If low mood returns every month and disrupts work or relationships, a doctor can help you match a treatment to how severe it is.

## At a glance

- SSRIs have the strongest evidence for premenstrual depression, including when taken only during the luteal phase, from symptom onset until a few days into your period ( [2]).
- Combined oral contraceptives have some supporting evidence, though it's less consistent than for SSRIs ( [4]). CBT is recommended in treatment guidelines too, though trial data specific to premenstrual depression is limited.
- Herbal and acupuncture trials for PMS and PMDD show improvement overall, but rarely measure depression on its own ( [3]).
- Bright light therapy's effect on premenstrual depression is small and statistically uncertain ( [5]).
- GnRH analogs with hormone add-back are reserved for depression that hasn't responded to other treatments ( [1]).

SSRIs have strong RCT support for premenstrual depression, including luteal-phase-only dosing (n=252). Hormonal, herbal and light-therapy options have weaker or mixed trial evidence. Severe, treatment-resistant cases have guideline support for GnRH analogs with hormone add-back, used only after other options fail.

## What is the best treatment for depression before your period?

The best PMS depression treatment depends on how severe your mood symptoms are and how many days they last each cycle.

- If your low mood eases within a few days of your period starting, you're likely dealing with PMS: common, and often helped by CBT, lifestyle steps and sometimes hormonal options.

- If it's severe enough to disrupt work, sleep or your closest relationships every month, it may fit the pattern of premenstrual dysphoric disorder (PMDD), a diagnosed condition with its own evidence-based treatments.

Selective serotonin reuptake inhibitors (SSRIs) have **the strongest evidence of any option** for premenstrual depression.

In a trial of 252 women with PMDD, sertraline taken only during the luteal phase worked better than placebo over six cycles. Women started it when symptoms began and stopped a few days after bleeding started ( [2]).

Depression before your period often comes from a sharper reaction to the normal drop in estrogen and progesterone before bleeding, not from abnormally low hormones themselves (Schmidt et al., 1998, NEJM; Dubey et al., 2017, Molecular Psychiatry). Hormonal contraceptives, CBT and herbal treatments all have some supporting evidence, though it's less consistent.

Here's how the main options compare:

| Option | What it does | Strength of evidence |
| --- | --- | --- |
| SSRIs, daily or luteal-phase only | Raises serotonin activity; eases low mood, irritability and anxiety | Strong (RCT, n=252) [2] |
| Combined oral contraceptives (drospirenone) | Flattens the hormone swing that can set off symptoms | Mixed (small RCT, n=25 completed) [4] |
| CBT | Builds new responses to the thoughts and reactions that worsen premenstrually | Guideline-recommended, limited trial data |
| GnRH analogs with hormone add-back | Stops ovulation and the hormone swing almost completely | Effective in treatment-resistant PMDD; reserved for severe, unresponsive cases [1] |
| Herbal options (vitex, St John's wort) | Varies by herb; most trials study PMS/PMDD symptoms together, not depression alone | Limited, inconsistent [3] |

## Do over-the-counter options help?

Over-the-counter pain relievers like ibuprofen or acetaminophen can ease cramps and headaches, but there's no trial evidence that they treat the depression itself. They're aimed at the physical side of PMS, not mood.

Supplements sold for PMS vary widely in what they contain and how much. Most haven't been tested specifically for depressive symptoms on their own, only as part of a broader PMS or PMDD symptom score ( [3]). That overlap makes it hard to say how much any single supplement helps your mood specifically.

What OTC options can realistically do:

- Pain relievers: ease cramps, headaches and breast tenderness, not low mood.

- General PMS supplements: tested mostly for symptom clusters, not mood alone.

- Herbal remedies with some trial data: covered in the next section.

If an OTC option isn't easing your mood within a cycle or two, that's worth mentioning to a doctor, since other options have more mood-specific evidence behind them.

Right now

What to do while you wait for treatment to help

- 1 Name what you're feeling without judging it: this is the depression that comes before your period.
- 2 Keep your routine small and doable: one shower, one meal, one short walk, nothing more.
- 3 Tell one person you trust that this week is harder, so you're not managing it alone.
- 4 Write down today's lowest moment so you can show a doctor the pattern later.
- 5 If a decision can wait until your period starts, let it wait.

Niyora walks you through this when it happens:

- Calm down

- See it another way

## What natural options have evidence?

A systematic review of 19 randomized trials looked at acupuncture and herbal medicine for PMS and PMDD, including vitex agnus-castus, St John's wort (Hypericum perforatum) and ginkgo biloba. Most of the trials reported improvement, but study quality varied and few separated depression from other premenstrual symptoms ( [3]). That overlap makes it hard to know how much of the benefit is specific to mood.

Bright light therapy has its own small evidence base: four crossover trials, 55 participants in total. The pooled effect on depressive symptoms was small, and the confidence interval crossed zero, meaning **the evidence doesn't confirm or rule out a real benefit** ( [5]).

If you want to try a herbal option, check with a pharmacist first. Some herbs interact with SSRIs or hormonal contraceptives, and dosing isn't standardized the way it is for medications.

## When should you start taking something?

Timing matters more for SSRIs than for most other options. In the sertraline trial, women started the medication at the first sign of luteal-phase symptoms. They stopped a few days after their period began, and repeated this every cycle for six months ( [2]). This symptom-onset dosing worked better than placebo over six cycles ( [2]).

For CBT, herbal options and lifestyle steps, there's no fixed start day. You can begin tracking and trying them at any point in your cycle, since the trial evidence doesn't point to one specific window.

A clinical guideline lays out treatment in order:

- Cycle regulation or luteal progesterone support

- Combined oral contraceptives

- SSRIs, taken daily or only during the luteal phase

- GnRH analogs with hormone add-back, for cases that don't respond to anything else ( [6], [1])

Tracking exactly when symptoms start and ease for two cycles helps you and a doctor decide on timing together. The [DRSP chart](https://niyora.com/tools/drsp-chart/) is the tool some of these trials, including the sertraline and oral contraceptive trials, used to confirm the pattern ( [2], [4]).

## When should you see a doctor?

Depression before your period that takes a week or more every month, or that gets in the way of work or your relationships, is worth a doctor's attention. Bring two months of daily notes on when it starts and when it eases to your appointment.

If you have thoughts of hurting yourself, tell your doctor right away. PMDD carries a high risk of these thoughts, and it's something a doctor needs to know about quickly ( [1]).

A doctor can also work out whether what you're feeling is premenstrual exacerbation: an existing depression that gets worse before your period but doesn't fully lift afterward ( [4]). It looks similar day to day, but the treatment plan is different, which is part of why two months of notes helps so much.

While you wait for an appointment, or for a treatment to start working, it helps to have somewhere to put the heaviest moments. Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what happened, see it from another angle, and decide what, if anything, you want to say, before you say it.

When to see a doctor

If this takes a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes using the DRSP chart.

## Questions people ask

What is the best treatment for depression before your period?

+

PMS depression treatment usually starts with lifestyle steps and CBT for mild symptoms, and SSRIs for more severe cases that fit a PMDD pattern. SSRIs have the strongest trial evidence, including when taken only in the luteal phase ( [2]). A doctor can help match the option to how severe and frequent your symptoms are.

Can birth control help with premenstrual depression?

+

Combined oral contraceptives containing drospirenone showed a trend toward improvement in a small trial of 25 women with depression that worsens before their period, but the difference from placebo wasn't statistically significant ( [4]). Birth control may still help some people, but it's less proven than SSRIs for premenstrual depression specifically.

Do herbal remedies like vitex treat PMS depression?

+

A systematic review of acupuncture and herbal medicine for PMS and PMDD, including vitex, found improvement in most trials, but the studies were small and didn't isolate depression from other symptoms ( [3]). PMS depression relief from herbs isn't ruled out, but the evidence doesn't single out mood as the main benefit.

How long does it take for premenstrual depression treatment to work?

+

SSRIs taken only in the luteal phase eased premenstrual depression over the trial's six treated cycles ( [2]); the trial didn't pin down how soon relief typically starts. CBT and lifestyle changes tend to take longer, often several cycles, since they build new responses rather than changing hormone-sensitive brain chemistry directly.

Is premenstrual depression the same as PMDD?

+

Not always. PMS depression is a milder mood dip in the week or two before your period that eases once it starts. PMDD depression is more severe, involves at least five symptoms including a mood symptom, and is confirmed by daily tracking over two cycles. Premenstrual exacerbation is different again: an existing depression that worsens but doesn't fully go away.
