# Sertraline side effects, including mood

Most people on sertraline notice nausea, headache or sleep changes in the first couple of weeks, and these usually ease. Sertraline can also shift mood, sometimes making you feel more anxious or restless before it helps. Weight change and sexual side effects are less predictable and can last longer, so a worsening effect is worth discussing with your doctor.

## At a glance

- Common side effects (nausea, headache, dry mouth, sleep changes) usually ease within one to two weeks.
- Sertraline can make mood feel more anxious or restless in the first weeks, a pattern called activation, before it settles.
- Weight change is possible but not universal, while sexual side effects can last as long as you keep taking it.
- Never stop sertraline abruptly: your doctor should guide any dose change to avoid discontinuation symptoms.
- If side effects don't ease after four to six weeks, interfere with daily life, or come with thoughts of hurting yourself, call your doctor that day.

Only one sertraline-specific trial is available: a small triple-blind RCT of sertraline 50mg daily for five weeks in women with PMS and resistant hypertension (n=40, 2016), well tolerated short-term but not a systematic side-effect study. The rest reflects general SSRI prescribing knowledge.

## What are the side effects of sertraline?

Sertraline is usually well tolerated, but most people notice at least one side effect, especially in the first couple of weeks of taking it.

Common, usually mild:

- Nausea or an upset stomach

- Headache

- Dry mouth

- Sweating more than usual

- Feeling drowsy, or trouble falling asleep

- Lower sex drive or trouble reaching orgasm

Less common:

- Diarrhea or constipation

- Dizziness

- Shaky hands

- Eating more than usual

Rare, but see a doctor the same day:

- Unusual bruising or bleeding

- A racing heart, high fever, confusion or stiff muscles (signs of serotonin syndrome)

- Racing thoughts, little need for sleep or reckless decisions (signs of a manic episode)

Most of these ease as your body adjusts to the medication. If you're taking sertraline for PMS or PMDD, your prescriber might suggest [taking it only in the two weeks before your period](https://niyora.com/treatments/sertraline-dosage/), which limits how many days you're exposed to side effects at all.

## Can sertraline affect your mood?

Yes. Sertraline can shift mood in both directions, which feels confusing when you're taking it for a mood problem in the first place.

In the first one to two weeks, some people feel more anxious, restless or on edge before they feel better. This is sometimes called activation, and it usually settles as the dose takes effect. [Sertraline for PMDD](https://niyora.com/treatments/sertraline-for-pmdd/) often works faster than it does for depression, but this early window still applies.

Antidepressants, including sertraline, carry a warning about a small increased risk of suicidal thoughts in people under 25, particularly in the first weeks of treatment or after a dose change. Less commonly, sertraline can trigger a manic episode in someone with an undiagnosed bipolar pattern.

If your mood swings feel sharper than usual, or you're reacting to small things in ways that surprise you, it helps to write down what happened and when.

Niyora is a PMS and PMDD support app, and its Moon feature lets you log what happened, see it from another angle and calm down before you decide what to say. Having that written down can make it easier to see later whether a mood change started with the medication or followed your cycle.

When to see a doctor

A mood that feels worse, more agitated or more hopeless than before you started is worth calling your doctor about, not waiting out.

## Can sertraline cause weight gain?

Weight change is possible, but it isn't the same for everyone. Some people gain a little weight over months of use, some lose a little, and many notice no change at all.

The mechanism isn't fully settled, and shorter trials often show only small average shifts in either direction. If you notice ongoing weight gain, mention it at a follow-up rather than waiting it out. Your doctor may adjust the dose or suggest a different option.

Appetite changes in the first few weeks, eating more or less than usual, are different from longer-term weight gain. Those early appetite shifts tend to settle once your body adjusts to the medication, while a slower weight trend over months is the one worth flagging.

## How long do sertraline side effects last?

Most of the common side effects, like nausea, headache and sleep changes, ease within the first one to two weeks as your body adjusts to the medication.

Sexual side effects, and for some people mild sweating, can last as long as you keep taking sertraline. These don't reliably fade with time the way nausea does.

This timeline is different from how long sertraline takes to work on mood or PMDD symptoms, which can take a few weeks to show its full effect. See [how long does sertraline take to work](https://niyora.com/treatments/how-long-does-sertraline-take-to-work/) for that separate timeline.

It's common to feel the side effects before you feel the benefit, which is part of why the first month on sertraline can be the hardest to get through. Knowing that order in advance can make it easier to stay the course if your doctor agrees it's worth it.

## When should you stop taking sertraline?

Stopping sertraline is a decision for you and your doctor, not something to do alone, even if a side effect is bothering you.

Stopping abruptly can cause discontinuation symptoms: dizziness, brain zaps, irritability or flu-like feelings. Your doctor will usually taper the dose down gradually instead of stopping all at once.

Call your doctor or go in the same day if you notice any of the following:

- Side effects that haven't eased after four to six weeks

- Side effects that interfere with work, sleep or your relationships

- A mood that feels worse, more agitated or more hopeless than before you started

- Thoughts of hurting yourself

If any of this is happening, bring a note of what changed and when, so your doctor can see the pattern alongside your cycle rather than guessing from memory.

## What are the alternatives to sertraline?

If sertraline's side effects outweigh its benefit for you, there are other options worth discussing with your doctor.

| Option | What it does | Evidence |
| --- | --- | --- |
| [Fluoxetine](https://niyora.com/treatments/fluoxetine-for-pmdd/) or [other SSRIs](https://niyora.com/treatments/ssri-for-pmdd/) | Same drug class, different side effect pattern | Moderate, several placebo-controlled trials for PMDD |
| Luteal-phase-only dosing | Limits sertraline exposure to the two weeks before your period | Moderate, recommended in treatment guidelines |
| Combined hormonal contraceptives | Steadies hormone swings across the cycle | Guideline-level recommendation; a related trial measured general contraceptive tolerability, not PMDD or mood symptoms |
| Cognitive behavioral therapy | Changes how you respond to symptoms day to day | Moderate, recommended alongside or instead of medication |
| Magnesium supplement | May ease some premenstrual anxiety for some people | Limited, a systematic review found inconsistent results across studies |

None of these is a straight swap. Switching SSRIs usually means a different mix of side effects rather than none at all, and non-drug options tend to help some symptoms more than others.

If you're earlier in the decision and haven't started sertraline yet, [sertraline for PMS](https://niyora.com/treatments/sertraline-for-pms/) and [sertraline for PMDD](https://niyora.com/treatments/sertraline-for-pmdd/) cover what the evidence says about whether it's likely to help you at all.

## Questions people ask

Does sertraline cause weight gain?

+

Sertraline can cause weight gain in some people, weight loss in others, and no change at all in many. The pattern isn't the same for everyone, and shorter trials usually show only small average shifts. If you notice ongoing weight change, mention it at your next appointment so your doctor can look at your options.

How long do sertraline side effects last?

+

Sertraline side effects like nausea, headache and sleep changes usually ease within the first one to two weeks as your body adjusts. Sexual side effects can last as long as you take it. If side effects haven't improved after four to six weeks, talk to your doctor about adjusting the dose or trying another option.

Can sertraline make PMDD mood swings worse before it helps?

+

In the first one to two weeks, sertraline can make some people feel more anxious or on edge, a pattern sometimes called activation. This usually settles as the dose takes effect. If PMDD mood swings feel sharper or your mood drops further than before, tell your doctor rather than waiting it out.

What's the difference between sertraline side effects and PMDD symptoms?

+

Sertraline side effects, like nausea or restlessness, tend to show up in the first weeks and often ease with time. PMDD symptoms follow your cycle, building in the week before your period and easing once it starts. Tracking both against your cycle helps tell them apart.

When should I call my doctor about sertraline side effects?

+

Call your doctor the same day if sertraline side effects haven't eased after four to six weeks, interfere with daily life, or come with a mood that feels worse, more agitated or more hopeless than before you started, or any thoughts of hurting yourself.
