# Magnesium for PMDD: does it work?

No. The only trial done specifically in women with confirmed PMDD, using intravenous magnesium, found it worked no better than placebo for mood symptoms. Evidence for oral magnesium easing PMS-related anxiety is mixed and mostly comes from small, lower-quality studies, and guideline treatments for PMDD, like SSRIs, certain hormonal contraceptives, and CBT, have stronger evidence behind them.

Mixed evidence

Strength of evidence Mixed Trials disagree, or the effect is small. - None - Limited - Mixed - Moderate - Strong

## At a glance

- The only trial run specifically in women with confirmed PMDD, giving magnesium by IV, found no mood benefit over placebo.
- Oral magnesium for PMS-related anxiety has mixed, mostly low-quality evidence across small trials.
- A 2019 meta-analysis of 13 studies found no consistent link between blood magnesium levels and PMS.
- A 2025 review of 31 PMS nutrient trials found vitamin B6, calcium, and zinc had more consistent effects than magnesium.
- No dose is validated for PMDD specifically. Related studies used 200 to 360 mg a day, and a doctor should weigh in before you start.

## Strength of evidence

Mixed evidence

- None
- Limited
- Mixed
- Moderate
- Strong

The only PMDD trial (IV magnesium) found no benefit over placebo for mood (4). PMS anxiety evidence is mixed, from small, low-quality trials, and a 2019 meta-analysis found no link between blood magnesium and PMS (1, 2, 6). A 2025 review found calcium, B6, and zinc had stronger evidence than magnesium (5).

## Does magnesium work for PMDD?

Right now, no strong evidence shows magnesium treats PMDD.

The most direct evidence comes from one randomized trial:

- It gave magnesium by IV to 17 women with confirmed PMDD and 14 without.

- Magnesium worked no better than placebo for mood symptoms (4).

Evidence for magnesium easing anxiety in women with PMS, a related but different experience, is more encouraging but still weak. Several small trials found a benefit, others found none, and the overall quality of the research is poor (1, 6).

**Magnesium isn't a guideline-recommended treatment for PMDD.** For where it might help with PMS specifically, see [magnesium for PMS](https://niyora.com/treatments/magnesium-for-pms/).

## What does the research say?

The picture gets more specific once you look at individual studies.

| Year | Study | Finding |
| --- | --- | --- |
| 2006 | RCT, IV magnesium, 17 women with PMDD vs 14 without | No better than placebo for mood symptoms; both groups retained similar magnesium levels (4) |
| 2017 | Systematic review, 18 studies | 4 of 7 PMS trials showed an anxiety benefit; samples were small and quality was rated poor (1) |
| 2016 | Review, same research group | Called the effect suggestive, asked for better trials (6) |
| 2019 | Meta-analysis, 13 observational studies | No consistent link between blood magnesium and PMS, weakening the idea that low magnesium causes symptoms (2) |
| 2025 | Review, 31 RCTs, 3,254 participants | Vitamin B6, calcium, and zinc had consistent effects; magnesium wasn't flagged as consistently effective (5) |
| 2024 | Review, anxiety and sleep (not PMDD) | Improved anxiety or sleep measures in most studies of people with anxiety or sleep disorders; none focused on PMDD (3) |

**The strongest PMDD-specific evidence points away from a mood benefit.**

## How do you take magnesium for PMDD?

There's no PMDD-specific dose backed by a trial.

**Guidelines recommend three first-line options for PMDD**, with a doctor deciding which fits you:

- SSRIs, sometimes taken only in the luteal phase

- Certain hormonal contraceptives

- CBT

See [PMDD treatment: what actually helps](https://niyora.com/treatments/pmdd-treatment/) for how those compare.

If you want to try magnesium anyway, research in related conditions used doses of roughly 200 to 360 mg a day:

- A migraine trial used 360 mg daily, from day 15 of the cycle through to the next period (7).

- Anxiety trials in PMS samples used comparable ranges (1).

Those studies weren't testing PMDD mood symptoms, so treat the numbers as a starting point, not a plan on their own. For more, see [magnesium dosage: how much and when](https://niyora.com/treatments/magnesium-dosage/).

## How long does it take to work?

Most studies that found a benefit measured it after several weeks of daily use, not after a single dose or cycle (4).

None of these trials followed PMDD's monthly pattern, so there's no solid data on whether taking magnesium only in the luteal phase changes that timeline.

**Give it at least one or two full cycles** before judging whether it's doing anything. Keep a daily note of your mood so you're not relying on memory alone. For more, see [how long does magnesium take to work](https://niyora.com/treatments/how-long-does-magnesium-take-to-work/).

## What are the side effects?

Magnesium's most common side effect, well documented outside these trials, is loose stools or stomach upset at higher doses.

- Doses used in research ranged from 200 to 360 mg a day (1, 7).

- None of the trials in women with PMS or PMDD reported serious harm.

- Going well above that range raises the risk of digestive side effects.

For a full list of what to watch for, including less common effects on mood, see [magnesium side effects, including mood](https://niyora.com/treatments/magnesium-side-effects/).

## Who should avoid it?

Magnesium is processed by the kidneys, so it can build up to harmful levels in anyone with kidney disease.

- It can interact with some antibiotics and other medications.

- Tell your doctor or pharmacist what you're already taking.

None of this caution is specific to PMDD. It applies anytime you add a supplement, and it's a short conversation to have before you start.

When to see a doctor

If this takes a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## What else helps alongside it?

Magnesium isn't the nutrient with the strongest evidence here.

The 2025 review of 31 trials found **vitamin B6, calcium, and zinc** had more consistent effects on PMS psychological symptoms than magnesium did (5). See [supplements for PMDD: what actually helps](https://niyora.com/treatments/supplements-for-pmdd/) for the full rundown.

Guideline treatments, SSRIs, certain hormonal contraceptives, and CBT, still have the strongest evidence for PMDD itself. A doctor is the one to weigh those against your history.

## Questions people ask

Does magnesium help with PMDD rage?

+

No trial has tested magnesium specifically for anger in PMDD. The closest study, an IV magnesium trial in women with confirmed PMDD, found no mood benefit over placebo (4). Evidence for magnesium easing PMS-related anxiety is mixed and comes from small, lower-quality trials (1, 6).

What type of magnesium is best for PMDD?

+

No study has compared magnesium forms for PMDD specifically, so there's no evidence-backed answer. Research trials in PMS and anxiety studies used plain oral magnesium, not a particular form (1, 6). Ask a pharmacist which form suits your stomach, since that's where differences mostly show up.

Is magnesium deficiency linked to PMDD or PMS?

+

A 2019 meta-analysis of 13 studies found no consistent link between blood magnesium levels and PMS (2). A 2006 trial found women with PMDD didn't retain magnesium differently than women without it, which argues against deficiency as the cause (4).

How much magnesium should I take for PMDD?

+

There's no PMDD-specific dose backed by research. Studies in related conditions used 200 to 360 mg a day (1, 7). Because PMDD guidelines recommend SSRIs, certain hormonal contraceptives, or CBT first, a doctor is best placed to weigh a magnesium dose against those options.

Can magnesium replace PMDD medication?

+

No. Magnesium hasn't been shown to work better than placebo in the one trial done in women with confirmed PMDD (4). Guideline treatments like SSRIs, certain hormonal contraceptives, and CBT have stronger evidence for PMDD, and a doctor is the right person to decide which fits you.
