# PMDD cravings: what they feel like and what helps

PMDD cravings are intense, specific urges for fat and sugar in the final week before your period, paired with a mood symptom like irritability or low mood. They come from a brain reacting strongly to the normal hormone drop before bleeding, not a lack of willpower. Certain contraceptives, chosen with a doctor, can ease the cravings themselves, and SSRIs can ease the mood symptoms that come with them.

Before your period

Builds after ovulation, peaks in the days before your period, and eases once bleeding starts.

## At a glance

- PMDD cravings are stronger and more specific than typical PMS cravings, and come paired with a mood symptom like irritability, sadness, or anxiety.
- They peak in the luteal phase, especially the final week before your period, and ease within a few days of it starting.
- Cravings happen because PMDD brains react differently to the normal hormone drop before a period, not because of abnormal hormone levels.
- Eating regularly, choosing a satisfying version of what you want, and naming the craving out loud can help in the moment.
- Daily tracking over two cycles helps a doctor tell a PMDD pattern apart from ordinary PMS cravings.

Hormonal contraceptives have RCT support for easing PMDD symptom clusters including cravings. SSRIs are recommended for PMDD generally. Vitamin D, exercise, and chasteberry help other PMS symptoms but show little specific effect on cravings in available trials.

## What do PMDD cravings feel like?

PMDD cravings can feel like a **sudden, specific pull** toward food, not ordinary hunger. Many women describe wanting one exact thing, chocolate, chips, or bread, and feeling restless or irritable until they get it.

Common experiences:

- A craving that shows up fast and feels urgent, not gradual

- Wanting fat and sugar together, like melted cheese or ice cream, more than plain carbs

- Eating past full and feeling disgusted with yourself afterward

- The craving layered on top of low mood, anxiety, or irritability, so it's hard to tell which came first

One small study of women with PMS found their desire for fatty foods rose sharply in the days before their period compared with after it, while desire for carbs alone stayed steady across the cycle (Appetite, 1999).

The craving is a brain responding to a hormone shift that happens every month, in a body that reacts to it more strongly than most, not a character flaw.

## Why does PMDD cause cravings?

Estrogen and progesterone fall sharply in the week before a period. That drop affects serotonin and the brain's reward circuits, which help regulate appetite and mood together.

People with PMDD don't have abnormal hormone levels. Their brains respond differently to the same monthly hormone changes everyone has (Schmidt et al., 1998, NEJM; Dubey et al., 2017, Molecular Psychiatry). That different reaction can show up as cravings, irritability, or both at once, since appetite and mood run through overlapping brain chemistry.

Serotonin plays a role in appetite regulation, which is one reason PMDD treatment targets serotonin pathways. SSRIs are a first-line PMDD treatment, but none of the trials cited here measured their effect on cravings specifically.

This is also why cravings in PMDD often arrive bundled with a mood symptom: rage, tearfulness, or anxiety sitting right next to the urge to eat. One study of women with PMS found their desire for fatty food was higher in the premenstrual phase than after it, even before any medication was involved (Appetite, 1999).

The craving is a real signal from a brain going through a real hormone shift, not a lack of willpower.

Right now

When a PMDD craving hits

- 1 Eat something now, don't wait on an empty stomach.
- 2 Choose a version of the craving that includes protein, like nuts with chocolate or yogurt with fruit.
- 3 Sit down with a plate instead of eating from the package while distracted.
- 4 Name it plainly: 'This is a PMDD craving, not me losing control.'

Niyora walks you through this when it happens:

## How is it different from PMS cravings?

PMS cravings are common and mild to moderate; most women notice some pull toward sweet or salty food before a period. PMDD cravings tend to be more intense, harder to resist, and tied to bigger mood swings.

| | PMS craving | PMDD craving |
| --- | --- | --- |
| Intensity | Noticeable, manageable | Strong, feels urgent or out of control |
| Timing | Luteal phase, eases with period | Final week before period, often severe |
| Mood link | Mild irritability possible | Paired with marked anger, low mood, or anxiety |
| Effect on eating | May eat a bit more | Can tip into eating past full or binge-like eating |

The difference isn't just degree. PMDD is a specific diagnosis in the DSM-5-TR: at least five symptoms, including one mood symptom, in the final week before a period, easing within days of it starting. A craving alone, even an intense one, isn't enough for a PMDD diagnosis on its own. It has to come with mood and functional symptoms, confirmed by daily tracking over at least two cycles.

If cravings are the main symptom and mood stays steady, ordinary PMS or premenstrual exacerbation is more likely than PMDD. For more, see [cravings before your period](https://niyora.com/symptoms/cravings-before-period/).

## When in your cycle is it worst?

PMDD cravings almost always worsen in the luteal phase, the one to two weeks between ovulation and your period. They tend to peak in the final week, when estrogen and progesterone are falling fastest.

For most women the pattern repeats: a steady appetite through the follicular phase, a shift starting a week or so after ovulation, and the strongest pull in the three to five days before bleeding starts. Cravings usually ease within a few days of the period beginning, which is one of the clues that points toward PMDD rather than a pattern present all month.

If cravings and mood symptoms show up every cycle in that same window, that pattern fits PMDD. If they show up at random times, or stick around all month, something else is more likely driving them, such as an eating pattern unrelated to the cycle or a mood condition that happens to worsen before the period.

Tracking the exact days can make the pattern clear. See [the PMDD symptoms list](https://niyora.com/symptoms/pmdd-symptoms/) for the full set of symptoms doctors look for, and [the luteal phase cravings explainer](https://niyora.com/cycle/luteal-phase-cravings/) for more on the timing.

## What can you do when it hits?

A craving this strong isn't something to push through on willpower alone. A few things help in the moment:

- Eat something, don't skip meals waiting it out. Hunger makes cravings and irritability worse.

- Choose a version of what you want that includes protein, like yogurt with honey instead of only sugar, so you feel satisfied instead of still searching for more.

- Put the food on a plate and sit down with it, instead of eating straight from the package while distracted.

- Name what's happening, in your head or out loud: 'This is a PMDD craving, not me losing control.' That can quiet the shame that often follows.

## Which treatments help with PMDD or PMS cravings?

No single treatment is approved specifically for cravings. Two contraceptive trials in women with diagnosed PMDD measured food craving directly; the other studies below were done in women with PMS. A doctor decides what's right for you based on your full symptom picture.

| Treatment | What it does | What the evidence shows |
| --- | --- | --- |
| SSRIs (daily or luteal-phase only) | First-line PMDD treatment, works on serotonin | Recommended for PMDD generally; not tested for craving specifically in these trials |
| Combined oral contraceptive with drospirenone | Suppresses ovulation, steadies hormone swings | Reduced the food-craving symptom cluster versus placebo across three cycles (p≤.0001; Contraception, 2011); an earlier trial of a similar drospirenone pill found a significant reduction in the same craving cluster (European Journal of Contraception, 2002) |
| CBT | Changes thought and behavior patterns around symptoms | Recommended for PMDD generally; not isolated for craving in available trials |
| Vitamin D | Daily supplement | Eased overall PMS severity and depression in a meta-analysis, but showed no measurable effect on craving itself (Journal of Clinical Medicine, 2026) |
| Diet pattern changes | Less simple sugar, fat, salt, and alcohol; more whole foods | Associated with lower PMS severity in observational research; more trials needed (Nutrients, 2024) |
| Chasteberry (Vitex agnus-castus) | Herbal supplement | Mixed results across PMS trials; limited evidence specific to cravings (Planta Medica, 2013) |

Exercise helped most PMS symptoms in one trial, like anxiety and tiredness, but didn't move food craving specifically (Archives of Gynecology and Obstetrics, 2018). A doctor or gynecologist can help you weigh these against your full pattern, not just the craving alone.

## How do you track it for your doctor?

PMDD is confirmed with daily ratings over at least two menstrual cycles. Tracking turns a feeling of 'I'm losing it every month' into a pattern a doctor can read.

What to log each day:

- The date and where you are in your cycle (days since your last period started)

- Any food cravings, how strong they felt, and whether you acted on them

- Mood symptoms: irritability, sadness, anxiety, or feeling out of control

- Whether symptoms eased once your period started

A simple paper calendar works, or [the PMS calculator](https://niyora.com/tools/pms-calculator/) to estimate your window. For the full diagnostic picture, see [PMDD explained](https://niyora.com/pmdd/).

When to see a doctor

See a GP or gynecologist if cravings and mood symptoms take up about a week of every month, or if you're having thoughts of hurting yourself. Bring two months of daily notes.

## Questions people ask

Are PMDD cravings a sign of weak willpower?

+

PMDD cravings aren't a sign of weak willpower. They come from a brain reacting strongly to the normal drop in estrogen and progesterone before a period, not from a lack of self-control. The urge itself is real, and the hormone shift behind it is real too, even when it's hard to manage in the moment.

Can PMDD cause binge eating?

+

PMDD can include eating past full or eating in a way that feels out of control in the days before a period, which some women describe as binge-like. If this happens every cycle alongside mood symptoms, it's worth tracking and raising with a doctor. See [the urge to binge before your period](https://niyora.com/moments/urge-to-binge/) for more on what helps in that moment.

Do vitamin D or chasteberry help PMDD cravings?

+

PMDD cravings don't respond much to vitamin D or chasteberry based on current evidence. A meta-analysis found vitamin D eased overall PMS severity and low mood but had no measurable effect on craving. Chasteberry shows mixed results across PMS trials, with limited evidence specific to cravings.

What's the difference between PMS cravings and PMDD cravings?

+

PMS cravings are common and usually mild, a bit more interest in sweet or salty food before a period. PMDD cravings are stronger, harder to resist, and arrive with a mood symptom like rage, despair, or anxiety, confirmed by at least two cycles of daily tracking.

When should I see a doctor about PMDD cravings?

+

PMDD cravings that show up every cycle and interfere with your week are worth bringing to a doctor. Take two cycles of daily notes on timing, mood, and eating to a GP or gynecologist, who can look at the full pattern and talk through options like SSRIs or certain contraceptives.
