# PMDD aches and pains: what it feels like and what helps

PMDD can bring real physical pain, joint aches, muscle soreness, headaches, breast tenderness and cramps, in the week before your period. Pain thresholds appear to drop premenstrually, which may explain why aches feel worse then. Heat, movement and rest can ease it now, and a doctor can help once you've tracked two cycles.

Builds after ovulation, peaks in the days before your period, and eases once bleeding starts.

## At a glance

- PMDD can bring joint pain, muscle aches, headaches, breast tenderness and cramps in the final week before your period, alongside the mood symptoms it's known for.
- Pain thresholds appear to drop in the premenstrual phase compared with the follicular phase, across pressure, heat, cold and muscle-pain tests (Pain, 1999).
- PMDD pain typically eases within a few days of your period starting and stays minimal the week after, unlike pain tied to an ongoing condition.
- Heat, gentle movement, over-the-counter pain relief and rest can ease the ache in the moment, though none of it treats PMDD itself.
- Tracking pain alongside mood for two cycles gives a doctor something concrete to work with if the pattern is disrupting your life.

A meta-analysis of pain-threshold studies shows pain sensitivity shifts across the cycle, with the premenstrual phase lowest. Treatment-specific evidence for PMDD pain (hypnotherapy, laser therapy, rose oil) comes from small trials only, so read it as early, not settled.

## What does PMDD aches and pains feel like?

PMDD pain is physical, not something you're imagining. In the week or so before your period, it can show up as joint aches, muscle soreness, headaches, breast tenderness, low back pain, or cramps that feel heavier than your usual ones.

Common ways it shows up:

- Aching or stiff joints, often hands, knees or hips

- Muscle soreness, like you overworked without exercising

- Headaches or migraines that build through the day

- Breast tenderness or swelling

- Low back pain or pelvic cramping

- A heavy, bruised feeling through your whole body

DSM-5-TR lists physical symptoms like breast tenderness and joint or muscle pain among the [eleven possible PMDD symptoms](https://niyora.com/symptoms/pmdd-symptoms/), though a diagnosis needs at least one mood symptom too. The pain is real even on days your mood feels steady.

## Does PMDD cause painful periods?

PMDD pain is timed to the days before your period. DSM-5-TR says PMDD symptoms, physical and emotional, should ease within a few days of bleeding starting and stay minimal the week after.

Painful periods, or dysmenorrhea, are different: that's cramping during your period itself, not before it. The two are tracked separately.

If your worst pain hits during your period rather than the week before, that points more toward dysmenorrhea. If it builds beforehand and eases once bleeding starts, that's the PMDD pattern. Either way, naming the pain specifically helps a doctor tell the two apart.

Right now

Right now, with the ache

- 1 Put heat on whatever hurts most, lower back, belly or jaw, for 15 to 20 minutes.
- 2 Take a short walk or stretch gently. Even five minutes helps stiff joints and muscles.
- 3 Take an over-the-counter pain reliever if that's something you'd normally use.
- 4 Lie down somewhere quiet and let your shoulders and jaw unclench.
- 5 Write down what hurts and how bad it is now, so you're not guessing later.

## Why does PMDD cause aches and pains?

Pain itself seems to feel different at different points in your cycle, independent of PMDD. A meta-analysis of sixteen studies found pain thresholds were **lowest in the premenstrual phase**, compared with the follicular phase, across pressure, heat, cold and muscle-pain tests [1].

That's a shift in how pain is perceived, not just more going wrong in your body. Estrogen and progesterone both drop sharply before your period, and PMDD looks like a different brain reaction to that normal drop, not abnormal hormone levels themselves.

Researchers studying hormone-sensitive brain conditions, including PMDD and menstrual migraine, think the same hormone swings that affect mood circuits may also change how the body processes pain signals [5].

The effect isn't limited to the premenstrual window. One study found people having gallbladder surgery during their follicular phase were more likely to report lingering pain months later than those operated on during their luteal phase, a sign that cycle phase can shape how pain settles in [6].

For more on the build-up through this part of your cycle, see why aches build in the luteal phase.

What the research says

A 1999 meta-analysis of sixteen studies found the premenstrual phase had the lowest pain thresholds of any cycle phase tested, across pressure, heat, cold and muscle pain [1].

## How is it different from PMS aches and pains?

PMS and PMDD can both bring aches and pains, and the pain itself can feel similar. What tells them apart is what comes with the pain and how much it disrupts your life.

| | PMS pain | PMDD pain |
| --- | --- | --- |
| Timing | Days before period, variable | Final week before period, consistent pattern |
| Mood symptom required | No | Yes, at least one |
| Typical severity | Mild to moderate | Often severe enough to disrupt daily life |
| How it's confirmed | Usually self-reported | Daily ratings over at least two cycles |

If the pain shows up with a mood symptom, irritability, low mood, anxiety or feeling rejected, and that combination disrupts your week every month, it's worth tracking closely. See [aches and pains a week before your period](https://niyora.com/symptoms/aches-and-pains-a-week-before-period/) for more on where the line sits.

## When in your cycle is it worst?

PMDD pain tends to build through the luteal phase, the two weeks after ovulation, and peaks in the final week before your period. That same meta-analysis found the premenstrual phase had the biggest drop in pain threshold of any phase studied, bigger than the rest of the luteal phase or ovulation [1].

For most people, the ache **eases within a few days of bleeding starting**. If pain is still bad a week into your period, or never lets up, mention that specifically to a doctor, since it may point to something other than PMDD, like dysmenorrhea or another pain condition.

If you're not sure which week you're in, [a PMS calculator can estimate your window](https://niyora.com/tools/pms-calculator/).

## What can you do when it hits?

When the ache sets in, you don't have to just push through it. A few things can take the edge off right then:

- Put heat on the sore spot, your lower back, belly or jaw, for 15 to 20 minutes

- Move gently. A short walk or light stretching can loosen stiff muscles and joints

- Take an over-the-counter pain reliever if that's something you'd normally reach for

- Rest somewhere quiet if your body is asking for it

If the heavier feeling is less about the ache itself and more about a fight you just had or something someone said, that's a different kind of weight to carry. Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what happened, see it from another angle, and draft a reply you can hold for 20 minutes before you send it.

Note

Heat, movement and over-the-counter pain relief can ease the ache in the moment. None of it treats PMDD itself, but getting through the next hour counts.

## Which PMDD treatments help aches and pains?

Guidelines for PMDD treat mood and physical symptoms together, not separately. They typically point to SSRIs (sometimes taken only in the luteal phase), certain hormonal contraceptives, and CBT, with a doctor deciding which fits you.

| Option | What it does | Evidence |
| --- | --- | --- |
| SSRIs | Can ease mood and physical symptoms together | Recommended in PMDD guidelines |
| Hormonal contraception | Can flatten some of the hormone swings people react to | Recommended for some; a doctor decides which kind |
| CBT | Eases distress around symptoms | Recommended alongside medical treatment |
| Hypnotherapy | Lowered pain intensity in one small trial of women with PMDD | Limited, single trial of 60 women (30 per group) [2] |
| Low-level laser therapy | Eased jaw and face pain in women with myofascial pain, though placebo worked just as well, and it didn't help those in their premenstrual phase | Limited, small trial, laser arm of 20 women (64 total) [3] |
| Damask rose (Rosa damascena) | Studied for menstrual pain across seven trials | No clear benefit shown [4] |

These are small, early trials, not proof that any of them fix PMDD pain specifically. A doctor can walk through which guideline-backed option makes sense for your pattern.

## How do you track it for your doctor?

A clear pattern is the most useful thing you can bring to an appointment. Track for at least two cycles:

- Note which days you have pain, and where: joints, muscles, head, back, breasts

- Rate it, even just mild, moderate or severe

- Note your mood that day too: irritability, low mood, anxiety or feeling rejected

- Mark the day your period starts

- Check whether pain and mood both ease within a few days of bleeding starting

Start with [what PMDD is](https://niyora.com/pmdd/) if you're still working out whether this fits, or look at [the eleven PMDD symptoms](https://niyora.com/symptoms/pmdd-symptoms/) to see the full list.

When to see a doctor

If this takes a week of every month, or the pain and mood together make it hard to function, see a GP or gynecologist and bring two months of daily notes.

## Questions people ask

Is PMDD pain the same every month?

+

PMDD pain can vary in strength from cycle to cycle, but the timing should stay the same, building in the final week before your period and easing within a few days of it starting. If the timing shifts constantly or pain continues all month, it may not fit the PMDD pattern.

Can PMDD cause joint pain?

+

Yes. Joint and muscle pain are listed among the physical symptoms that can go along with PMDD in DSM-5-TR, usually alongside breast tenderness, bloating or headaches. PMDD joint pain tends to ease once your period starts, which can help you tell it apart from an ongoing joint condition.

Does PMDD pain mean something is wrong with my joints or muscles?

+

Not necessarily. PMDD pain often reflects a cyclical shift in how your body senses pain, not new damage to your joints or muscles. If swelling, redness or pain outside the premenstrual week shows up, mention it to a doctor separately.

Can painkillers help with PMDD aches and pains?

+

Over-the-counter pain relievers can ease PMDD aches and pains in the moment, the same way they'd help any muscle or joint pain. They don't change the underlying hormone sensitivity behind PMDD, so they're a short-term tool for the ache itself.

How long does PMDD pain usually last?

+

PMDD pain typically builds through the final week of the luteal phase and eases within a few days of your period starting, staying minimal for the rest of the cycle. Pain that lingers well into your period or doesn't let up points toward a separate cause.
