# PMDD physical symptoms: what it feels like and what helps

PMDD brings physical symptoms, bloating, breast tenderness, headaches, joint pain and fatigue, in the week before your period, alongside the mood symptoms that define the diagnosis. They happen because of a different brain response to the normal hormone drop before bleeding, not abnormal hormone levels. SSRIs, some hormonal contraceptives and CBT are the treatments with the most evidence behind them.

## At a glance

- PMDD physical symptoms (bloating, breast tenderness, headaches, fatigue, joint and muscle pain) show up in the week before your period, alongside mood symptoms.
- They happen because of a different brain response to the normal drop in estrogen and progesterone, not abnormal hormone levels.
- PMS and PMDD can share the same physical symptoms; about 3% to 10% of people with PMS symptoms may meet full criteria for PMDD.
- SSRIs like fluoxetine have the strongest evidence for easing PMDD's physical symptoms; some hormonal contraceptives and vitamin D show smaller benefits.
- Tracking symptoms daily for at least two cycles is what turns a pattern into something a clinician can act on.

## Strength of evidence

One placebo-controlled RCT supports SSRIs for PMDD physical symptoms, and a large contraceptive trial and Cochrane review show some benefit from hormonal options. Smaller trials on vitamin D and zinc show promise but have limited or very low certainty evidence.

## What do PMDD physical symptoms feel like?

PMDD brings real physical symptoms in the week or two before your period. They layer on top of the irritability, anxiety or low mood that define the diagnosis.

Common PMDD physical symptoms include:

- Breast tenderness or swelling

- Bloating or a feeling of weight gain

- Headaches

- Joint or muscle aches

- Fatigue that feels heavier than usual tiredness

- Changes in appetite or specific cravings

- Trouble sleeping, even when you're exhausted

These show up alongside at least one core mood symptom, and they **ease within a few days of your period starting**.

## Why does PMDD cause physical symptoms?

Estrogen and progesterone fall sharply in the days before your period, for everyone who ovulates. Those hormones affect fluid balance, pain sensitivity and the brain chemicals that shape mood, which is why bloating, breast tenderness and headaches so often arrive with irritability or sadness.

What sets PMDD apart isn't abnormal hormone levels. It's **a different brain response** to that normal hormonal drop, which can make both the physical and emotional symptoms land harder than they do for most people.

Body composition may play a role too. A systematic review found higher BMI and more central body fat linked to more severe PMS symptoms (systematic review, 2023). This research looked at PMS, not PMDD specifically, so it's not clear the link holds the same way.

Right now

What to do right now when PMDD physical symptoms hit

- 1 Put a heating pad on your lower belly or back for cramps and muscle aches.
- 2 Lie down with your legs propped against a wall for 15 minutes to ease bloating.
- 3 Take a short, gentle walk if your joints feel stiff; moving can help more than resting still.
- 4 Drink a glass of water instead of reaching for something salty or sugary.
- 5 Set a timer for 20 minutes of rest before you decide what else you need to do today.

## How is PMDD different from PMS physical symptoms?

PMS and PMDD can bring the same physical symptoms: bloating, breast tenderness, headaches, fatigue. What separates them is severity, the required mood symptom, and how much the week disrupts your life.

| | PMS | PMDD |
| --- | --- | --- |
| When it happens | 1 to 2 weeks before your period | Final week before your period |
| Physical symptoms | Common, mild to moderate | Same symptoms, often felt more severely |
| Mood symptom required | No | Yes, at least one core mood symptom |
| How many are affected | Most people who ovulate get some symptoms | About 3% to 10% of people with PMS symptoms may meet full PMDD criteria (Cochrane review, 2017) |

For the fuller picture on non-PMDD physical symptoms, see physical symptoms before your period.

## When in your cycle are PMDD physical symptoms worst?

PMDD physical symptoms follow the same pattern as the mood symptoms. They build through the luteal phase, the two weeks between ovulation and your period, and peak in the final week before bleeding starts.

For a diagnosis, DSM-5-TR requires symptoms to ease within a few days of your period starting and stay minimal through the week after it ends. If your physical symptoms linger all month instead, something else, like premenstrual exacerbation of another condition, may be going on.

Tracking where you are in your cycle makes the pattern visible. The [PMS calculator](https://niyora.com/tools/pms-calculator/) can give you an estimated window to watch.

## What can you do when PMDD physical symptoms hit?

There's no instant fix, but a few things can take the edge off while you wait for the luteal phase to pass.

- Heat on your lower belly or back eases cramping and muscle aches

- Lying down with your legs propped up reduces the heavy, swollen feeling of bloating

- A short, gentle walk can loosen stiff joints more than resting completely still

- Water instead of salty or sugary food can ease some water retention

- A short nap helps when the fatigue feels bone-deep

These ideas can help in the moment. They don't treat PMDD, and the week of symptoms is still real even on the days the coping helps.

Niyora is a PMS and PMDD support app. If the physical exhaustion comes with a fight you can't shake or a reply you're not sure about, Niyora's Moon lets you write what happened, see it from another angle and draft a reply you can hold for 20 minutes before you send it.

In the moment

If the physical symptoms come with a heavy moment, writing it down somewhere first can give you a little distance before you respond.

## Which PMDD treatments help physical symptoms?

A clinician decides on treatment, based on your symptoms and history. PMDD research is thinner than PMS research. Fluoxetine is the only option below tested specifically in women with diagnosed PMDD; the rest come from PMS trials and may or may not carry over:

| Option | What it may help | Evidence |
| --- | --- | --- |
| SSRIs (e.g. fluoxetine) | Breast tenderness, bloating, headache | RCT in 320 women with PMDD, BJOG 2001 |
| Hormonal contraceptive (estetrol/drospirenone) | Pain, water retention, low mood | RCT in 1,553 women with PMS, not PMDD, 2024 |
| Transdermal estrogen | Suppressing the hormone swings behind symptoms | Cochrane review of PMS trials, 2017, evidence limited |
| Vitamin D | Overall severity, physical symptoms | Meta-analysis of 5 PMS trials, 2026, very low certainty |
| Zinc | Headache, bloating, breast tenderness | Small trial in PMS, not PMDD, 2023 |

SSRIs are usually tried first for PMDD and can be taken daily or only in the luteal phase, depending on what your doctor recommends. CBT is also part of standard PMDD care, mainly for mood symptoms, though easing those can make the physical days feel less overwhelming too.

## How do you track PMDD physical symptoms for your doctor?

PMDD is confirmed with daily symptom ratings over at least two menstrual cycles, not a single conversation. Track both physical and mood symptoms, note the day of your cycle, and mark when each symptom starts and ends.

What to write down each day:

- Physical symptoms: bloating, breast tenderness, headache, fatigue, pain

- Mood symptoms: irritability, anxiety, low mood, rejection sensitivity

- Severity, on a simple scale like 1 to 3

- Whether your period has started that day

See [PMDD symptoms: the 11 DSM-5 symptoms, explained](https://niyora.com/symptoms/pmdd-symptoms/) for the full symptom list clinicians use, and try [this quiz](https://niyora.com/quiz/) to see how much PMS or PMDD may be affecting you.

Bring two months of these notes to your next appointment. That's what turns a pattern into something a clinician can act on.

When to see a doctor

If this takes a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## Questions people ask

Are physical symptoms part of PMDD or just PMS?

+

PMDD physical symptoms are common, but DSM-5-TR doesn't count them toward the five required symptoms unless paired with a core mood symptom. Still, bloating, breast tenderness, fatigue and headaches often show up alongside the mood changes in the week before your period and ease within days of it starting.

What is the most common physical symptom of PMDD?

+

Breast tenderness and bloating are among the most frequently reported PMDD physical symptoms, often alongside fatigue, headaches and joint or muscle pain. Symptoms vary by person and cycle, so what's worst for you one month may be different the next.

Can PMDD cause physical pain?

+

Yes. PMDD can bring joint and muscle aches, headaches, breast tenderness and cramping in the days before your period, on top of mood symptoms. The pain usually eases within a few days of your period starting, which helps separate it from conditions present all month.

Can diet or supplements help PMDD physical symptoms?

+

A meta-analysis of five small trials (436 women) found vitamin D eased overall PMS severity and physical symptoms, though certainty is rated very low. A small trial found zinc reduced bloating and breast tenderness. Both trials studied PMS, not diagnosed PMDD, so it's not clear the effect is the same for PMDD. Treat them as small additions alongside standard PMDD care, not a replacement for SSRIs or CBT.

Can physical symptoms alone point to PMDD?

+

PMDD diagnosis always requires at least one core mood symptom alongside the physical ones, confirmed with daily tracking over two cycles. Physical symptoms on their own, without a mood symptom, point to PMS rather than PMDD.
