# Symptom-Free Week: what it means

The symptom-free week is the stretch of days right after your period, usually the first week of the follicular phase, when PMS and PMDD symptoms should ease almost completely. For a PMDD diagnosis, DSM-5-TR requires this week to show real relief, confirmed with daily tracking, not memory. See the full picture on the [PMDD](/pmdd/) page.

## At a glance

- The symptom-free week is the follicular week after your period, when PMS and PMDD symptoms should ease almost completely.
- DSM-5-TR requires symptoms to be minimal or absent that week for a PMDD diagnosis to apply.
- If symptoms never fully clear, it may be premenstrual exacerbation of another condition, not PMDD.
- Confirming the pattern takes daily tracking across at least two cycles, not memory of one premenstrual week.
- Treatment choices often depend on whether that clear week actually happens.

## What is the symptom-free week?

The symptom-free week is the stretch of days right after your period, usually the first week of the follicular phase, when PMS and PMDD symptoms should ease almost completely.

For PMS, this week is simply the far side of a pattern that already fades once bleeding starts: symptoms recur during the luteal phase and reduce by the end of menstruation [2].

For PMDD, this week carries more weight. DSM-5-TR requires symptoms to be **minimal or absent** in the week after your period for the diagnosis to apply. Without that stretch of real relief, what looks like PMDD may be premenstrual exacerbation instead, an existing condition (depression, anxiety, ADHD) that gets worse before your period but never fully lifts.

| Pattern | Symptom-free week? | What it suggests |
| --- | --- | --- |
| PMS | Mostly, eases once your period starts | A common premenstrual pattern |
| PMDD | Required: minimal or absent symptoms after menses | A diagnosis confirmed by daily tracking |
| Premenstrual exacerbation | Rarely full, symptoms persist though lighter | An existing condition, worse before your period |

Understanding this pattern is part of how [PMDD](https://niyora.com/pmdd/) gets diagnosed, alongside what happens during your [luteal phase](https://niyora.com/luteal-phase/).

## What does the research say about it?

A crossover trial of people with past-month suicidal thoughts found 59% met every DSM-5 PMDD symptom criterion except one: full follicular symptom remission. Their symptoms never fully cleared in that supposed symptom-free week [5]. That detail shows how common this pattern can be without the one full clear week PMDD technically requires.

Hormone research helps explain why that week exists at all.

In women with PMDD, plasma allopregnanolone, a byproduct of progesterone, rises from the follicular into the luteal phase, tracking the shift from easier days into harder ones [3].

Estrogen and progesterone fall again before your period, and allopregnanolone, which tracks progesterone, likely falls with them.

None of this means your hormone levels are unusual. [What causes PMDD](https://niyora.com/questions/what-causes-pmdd/) covers why some brains react more strongly to the same monthly hormone shifts other people barely notice.

## Why does it matter for treatment and coping?

Confirming a true symptom-free week changes what gets treated and how.

How it gets confirmed:

- Track mood and physical symptoms daily, not from memory.

- Keep the log for at least two full menstrual cycles.

- Check whether the week after your period is genuinely clear, not just better.

What that confirms:

- A clear week each cycle fits a premenstrual pattern like PMS or PMDD.

- No clear week points toward premenstrual exacerbation of an ongoing condition.

The difference shapes treatment.

Trials have tested sertraline dosed only from symptom onset through the first days of a period, rather than every day, with benefit for PMDD [4]. Combined oral contraceptives have some evidence for easing premenstrual depressive symptoms, and a doctor weighs this against your overall pattern, including whether a clear week shows up [1].

Without a clear week, doctors often treat the underlying condition daily rather than only premenstrually, but a doctor decides what fits your pattern.

If this takes a week of every month, or your symptoms never fully lift, see a GP or gynecologist and bring two months of daily notes.

In the meantime, if you want somewhere to put a hard evening before you react, Niyora is a PMS and PMDD support app. It estimates your PMS window and gives you space to write out what happened.

## What related terms should you know?

A few terms tend to come up alongside the symptom-free week:

- Luteal phase: the back half of your cycle, after ovulation, when PMS and PMDD symptoms build ([the luteal phase](https://niyora.com/luteal-phase/))

- Corpus luteum: the structure that makes progesterone after ovulation, and whose breakdown triggers the hormone drop before your period ([corpus luteum](https://niyora.com/glossary/corpus-luteum/))

- Premenstrual exacerbation: an existing condition that worsens before your period without ever fully clearing, the main thing a true symptom-free week rules out

For how people actually talk about this stretch of the month, see [what PMS means in slang](https://niyora.com/glossary/pms-slang/). For the evidence behind these terms, [Niyora's research page](https://niyora.com/research/) summarizes the studies.

## Questions people ask

What exactly is the symptom-free week?

+

The symptom-free week is the stretch of days after your period, usually the first week after bleeding starts, when PMS and PMDD symptoms should fully ease. For a PMDD diagnosis, DSM-5-TR requires this week to be close to symptom-free, confirmed with daily ratings across at least two cycles.

How long does the symptom-free week last?

+

The symptom-free week usually lasts through the first week of the follicular phase, between your period ending and the approach of ovulation. Symptoms then build again through the luteal phase, typically returning in the week or so before your next period starts.

What if my symptoms never fully go away?

+

If the symptom-free week never fully arrives and symptoms don't clear after your period, that doesn't automatically mean PMDD. It may point to premenstrual exacerbation, where an existing condition worsens before your period but never fully lifts. A doctor can help sort out which pattern fits.

How do doctors confirm a true symptom-free week?

+

Doctors confirm a symptom-free week with daily symptom ratings kept across at least two menstrual cycles, not with memory of a single month. The pattern needs to show real relief in the week after bleeding starts before a PMDD diagnosis is confirmed.
