# Progressive muscle relaxation: what it means

Progressive muscle relaxation (PMR) is a technique where you tense, then release, one muscle group at a time to lower physical tension. Several small trials in women with PMS link regular PMR practice to lower symptom scores and less anxiety. It's a coping tool for the days before your period, not a treatment for PMDD on its own.

## At a glance

- Progressive muscle relaxation (PMR) means tensing a muscle group for a few seconds, then releasing it, moving through the body in order.
- Randomized trials in women with PMS link several weeks of PMR practice to lower symptom scores than no intervention.
- PMR can ease the physical tension, pain, and sleep problems that often show up in the luteal phase, PMS, and PMDD.
- PMR doesn't change the hormone shifts behind PMDD, so it works alongside, not instead of, treatments like SSRIs, certain hormonal contraceptives, or CBT.
- A typical PMR session runs 10 to 20 minutes and can be done with a guide or recorded script.

## What is progressive muscle relaxation?

Progressive muscle relaxation (PMR) is a technique for lowering physical tension. Each cycle has three parts:

- Tense one muscle group for about five seconds.

- Release it completely.

- Notice the difference between tight and loose before moving to the next muscle group.

You repeat this through the body in order, usually starting at your feet and ending at your face.

Typical sequence:

- Tense your feet and calves for five seconds, then release.

- Move up to your thighs and glutes, tense, then release.

- Work through your stomach, chest, hands, arms, shoulders, and face.

- Finish by noticing how your whole body feels compared to when you started.

Sessions usually run 10 to 20 minutes. Some people use a recorded script or app to guide the pacing.

## How does PMR relate to PMS and PMDD?

In the [luteal phase](https://niyora.com/luteal-phase/), the one to two weeks before your period, falling estrogen and progesterone can bring physical tension, pain, and trouble sleeping alongside mood changes. Muscle tightness in your neck, shoulders, or jaw often tracks with this window.

PMR targets that physical layer directly. It doesn't change the hormone shifts behind [PMDD](https://niyora.com/pmdd/). People with PMDD react differently to normal hormone changes, not to abnormal levels, and PMR can still lower the body tension that adds extra strain to the days before your period.

That's different from premenstrual exacerbation, where an existing condition worsens before your period but doesn't ease once it starts. PMR can help with physical tension in either pattern, even though it isn't a fix for the underlying condition.

## What does the research say?

Several small randomized trials have tested PMR in women with PMS. Most ran for four to eight weeks, with daily or near-daily practice.

| Study | Participants | What it found |
| --- | --- | --- |
| Women & health, 2024 | 68 university students | Yoga and yoga+PMR groups had lower PMS scores than the PMR-only and control groups; the standalone PMR arm did not differ significantly from control in this trial |
| Archives of psychiatric nursing, 2024 | 79 women | The PMR group's symptom score was lower than the control group's after 8 weeks, and the PMR group's own pre-to-post drop was statistically significant (p < 0.001) |
| BMC women's health, 2025 | 106 women | The PMR group had lower PMS and violence-tendency scores than control after 8 weeks |
| Medicine, 2023 (registered protocol) | PMR vs. myofascial release vs. control | Protocol to compare symptoms, pain, sleep, and quality of life; results not yet published |
| Women & health, 2026 (systematic review) | 1,598 participants across 22 studies | Mindfulness and relaxation methods, including PMR, lowered physical and psychological symptoms |

The pattern across these trials is consistent. Regular PMR practice tracks with lower PMS symptom scores than no intervention at all.

Trial sizes are small, and one recruited only university students, so the evidence is moderate rather than definitive. None of these trials recruited women with a confirmed PMDD diagnosis, so the findings apply most directly to PMS.

## Why does PMR matter for treatment or coping?

PMR is a coping tool, not a treatment for PMDD. For PMDD, guidelines point to SSRIs (which can work taken only in the luteal phase), certain hormonal contraceptives, and CBT, with a doctor deciding what fits. PMR can sit alongside any of those, lowering physical tension on nights when a tight jaw or restless legs make everything else harder.

If you want something to do in the moment instead of a longer practice, Niyora is a PMS and PMDD support app. Its Moon feature helps you slow down and decide what to say before you react, which can sit well alongside a few minutes of PMR when your body feels wound up.

## Questions people ask

What is progressive muscle relaxation used for?

+

Progressive muscle relaxation is used to lower physical tension and stress by tensing and releasing muscle groups one at a time. People use it for general stress, pain, and sleep problems, and several small trials link it to lower premenstrual syndrome symptom scores after four to eight weeks of practice.

Can progressive muscle relaxation help PMDD?

+

Progressive muscle relaxation can ease the physical tension that often comes with PMDD, like a tight jaw or trouble sleeping, but it doesn't change the hormone shifts behind the condition. Guidelines point to SSRIs, certain hormonal contraceptives, and CBT as treatment, decided with a doctor.

How long does it take to see results from PMR?

+

Most trials testing progressive muscle relaxation for PMS ran participants through four to eight weeks of daily or near-daily practice before measuring symptom scores. Some people notice less physical tension within a single session, but the research measuring premenstrual symptoms used weeks of regular practice.

Is progressive muscle relaxation the same as mindfulness?

+

No. Progressive muscle relaxation is a specific technique of tensing and releasing muscle groups in sequence, while mindfulness is a broader practice of noticing thoughts and sensations without reacting. A systematic review grouped both under relaxation-based approaches for premenstrual syndrome, but they work differently.
