# Progestin vs progesterone: what it means

Progesterone is the hormone your ovaries make naturally after ovulation. Progestin is a lab-made compound that acts on the same receptors, used in birth control and some PMS treatments. Trials of both have shown little benefit for PMS on their own, while PMDD research points more to how your brain reacts to the hormone shift than to the hormone level itself.

## At a glance

- Progesterone is the natural hormone your ovaries make after ovulation; progestin is a synthetic version used in birth control pills, IUDs, and some treatments.
- Progestogen is the umbrella term for both the natural hormone and its synthetic versions.
- A BMJ systematic review of 531 women found no clinically important difference between progesterone and placebo for PMS.
- A newer progesterone receptor modulator showed a 93% response rate for PMDD versus 53% on placebo, suggesting the brain's reaction matters more than hormone levels.
- Guidelines don't recommend progesterone or progestins alone for PMS; SSRIs, certain hormonal contraceptives, and CBT are what's currently recommended for PMDD, with a doctor deciding what fits.

## What is progestin vs progesterone?

Progesterone is a steroid hormone your ovaries make naturally, mostly after ovulation. The [corpus luteum](https://niyora.com/glossary/corpus-luteum/) produces it during the [luteal phase](https://niyora.com/luteal-phase/), the one to two weeks before your period.

Progestin is different: it's a synthetic compound made to act on the same receptors as progesterone. You'll find progestins in:

- Birth control pills

- Hormonal IUDs

- Some treatments for PMS or heavy bleeding

**Progestogen** is the umbrella term covering both natural progesterone and synthetic progestins. Researchers use it when a study includes both types, since the two act on similar receptors even though their chemistry differs.

## How does it relate to PMS and PMDD?

Progesterone rises through the luteal phase, then falls sharply in the days before your period. One of its byproducts, [allopregnanolone](https://niyora.com/glossary/allopregnanolone/), acts on GABA receptors in the brain, the same system calmed by anti-anxiety medications.

For most people, that drop passes without much disruption. [PMDD](https://niyora.com/pmdd/) seems to involve a different brain reaction to that normal hormone shift, not an abnormal amount of progesterone itself (see [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/)).

Because progestins act on the same receptors, how one affects your mood depends on a few things:

- Which specific progestin it is (they're not interchangeable)

- The dose and how it's delivered (pill, IUD, implant)

- Your own sensitivity to hormone shifts

That's why a progestin that helps one woman's PMS can feel worse for another.

## What does the research say?

Clinical trials have tested progesterone and progestins for PMS directly, and the results are mixed at best.

| Treatment | Studied for | What trials found |
| --- | --- | --- |
| Progesterone (oral or suppository) | PMS | No clinically important difference versus placebo (BMJ systematic review, n=531) |
| Progestogens (synthetic) | PMS | Small statistical improvement, not considered clinically meaningful (BMJ systematic review, n=378) |
| Progestins | PMS pain | No PMS-specific trials found; evidence comes from endometriosis and fibroid studies instead (2025 systematic review, n=2745) |
| Selective progesterone receptor modulator | PMDD | 93% treatment response versus 53% on placebo, with measurable changes in brain reactivity during anger (2021 RCT, n=30) |

Newer compounds that modulate the progesterone receptor, rather than simply adding more progesterone, show more promise for PMDD than raising hormone levels alone.

## Why does it matter for treatment, and what else should you know?

Guidelines don't recommend plain progesterone or progestins as a stand-alone PMS or PMDD treatment, based on the trial evidence above. Current guidelines for PMDD instead point to:

- SSRIs, sometimes taken only in the luteal phase

- Certain hormonal contraceptives

- Cognitive behavioral therapy (CBT)

A doctor decides what combination fits your situation and history.

Some hormonal contraceptives contain progestins and can still ease symptoms for some women, even though progestin alone hasn't shown clear benefit in PMS trials. The combination, dose, and your own biology all play a part.

The [PMS and PMDD research library](https://niyora.com/research/) has more on what's been tested, including the trials above.

## Research on the pill

502 studies in the library look at the pill in PMS and PMDD. They were published from 1970 to 2026, and each one has a short summary of what was tested and found.

- [Systematic review, 2026Does continuous paroxetine or birth control work better for PMDD?Both antidepressants and birth control pills are effective for PMDD. Continuous paroxetine (an antidepressant) and DROS/EE 24/4 (a specific birth control) showed the strongest effects in studies reviewed, though the evidence comes from relatively few studies.Strong study design](https://niyora.com/research/papers/comparison-of-premenstrual-dysphoric-disorder-treatment-with-antidepressants-and-2026/)

## Questions people ask

Is progestin the same as progesterone?

+

No. Progesterone is the hormone your ovaries make naturally after ovulation. Progestin is a lab-made compound that acts on the same receptors but isn't molecularly identical. Progestins are found in birth control pills, hormonal IUDs, and some PMS treatments; progesterone itself is sometimes given as a pill or suppository.

Does progesterone help with PMS?

+

Trials haven't shown it does. A BMJ systematic review of 531 women found no clinically important difference between progesterone and placebo for PMS (BMJ, 2001). Progestogens showed a small statistical improvement that wasn't considered clinically meaningful in that same review (n=378). A separate Cochrane review of two small trials (280 women) couldn't pool the results (Cochrane, 2012).

Why do progestins show up in PMDD treatment if they don't help PMS?

+

Guidelines don't recommend plain progesterone or progestins as a stand-alone PMS or PMDD treatment, based on the trial evidence above. Some hormonal contraceptives, which contain progestins, can still help some women; a doctor weighs that option alongside SSRIs and CBT, the treatments current PMDD guidelines support.

What's a progestogen?

+

Progestogen is the umbrella term covering both natural progesterone and synthetic progestins. You'll see it in research papers that group the two together, since they act on similar receptors even though they aren't chemically the same, and that difference can affect mood too.

Can progesterone levels explain why PMDD feels worse than PMS?

+

Not on their own. Schmidt et al. (1998, NEJM) and Dubey et al. (2017, Molecular Psychiatry) found PMDD involves a different brain reaction to normal hormone shifts, not having more or less progesterone than anyone else. That's why tracking what helps you can matter more than measuring hormone levels.
