# Mittelschmerz: what it means

Mittelschmerz is the medical term for the one-sided pain some people feel around ovulation, about two weeks before a period. It's separate from PMS and PMDD, which show up later in the luteal phase. Noticing it can help you tell where you are in your cycle.

## At a glance

- Mittelschmerz means "middle pain" in German and refers to the pain some people feel at ovulation.
- It happens midcycle, about two weeks before a period, not during the PMS or PMDD window.
- The ache is usually brief, lasting minutes to about a day or two, and can switch sides from cycle to cycle.
- It's a normal part of ovulation, not a symptom of PMS or PMDD, though it can help you track your cycle.

## What is mittelschmerz?

Mittelschmerz is the medical term for the one-sided pain some people feel around ovulation, roughly two weeks before a period. The word comes from German for "middle pain," because it shows up in the middle of the cycle rather than near the period itself.

It usually shows up as:

- A dull ache or a sharp twinge low on one side of your belly

- Pain that lasts a few minutes to about a day or two

- A switch in sides from month to month, depending on which ovary releases the egg

- Sometimes light spotting or a small change in discharge around the same time

Not everyone notices ovulation at all, and mittelschmerz on its own isn't a problem. Doctors think it comes from the follicle stretching right before it releases an egg, or from the small amount of fluid that follows.

## How does mittelschmerz relate to PMS and PMDD?

Mittelschmerz happens at ovulation. PMS and PMDD happen later, in the [luteal phase](https://niyora.com/luteal-phase/), the one to two weeks between ovulation and your period. They're different events in the same cycle, not the same thing.

| Term | When it happens | What it feels like | Part of PMS or PMDD? |
| --- | --- | --- | --- |
| Mittelschmerz | Midcycle, at ovulation | One-sided pelvic twinge or ache | No |
| PMS | Final one to two weeks before a period | Mood changes, bloating, breast tenderness | By definition |
| PMDD | Final week before a period | Severe mood symptoms, see [PMDD, explained](https://niyora.com/pmdd/) | By definition |

Some people use mittelschmerz as a marker. Once that twinge passes, they know the luteal phase, and the window where PMS or PMDD symptoms can build, is starting. For how estrogen and progesterone shift across these phases, see [menstrual cycle hormones, phase by phase](https://niyora.com/cycle/menstrual-cycle-hormones/).

## Why does it matter for treatment or coping?

Knowing mittelschmerz is separate from PMS or PMDD matters because it changes what you're treating. Pain at ovulation usually needs nothing more than rest and a way to manage the ache. Mood or physical symptoms that show up later, as the [corpus luteum](https://niyora.com/glossary/corpus-luteum/) forms and then breaks down, have a different cause.

What can help with mittelschmerz pain right now:

- A heating pad or warm bath on the side that hurts

- An over-the-counter pain reliever, if your doctor says it's fine for you

- Rest if the ache is sharp enough to need it

When to see a doctor

If the pain is severe, lasts more than two or three days, or comes with fever or heavy bleeding, that's not typical mittelschmerz. See a GP or gynecologist.

## What related terms should you know?

A few terms often come up alongside mittelschmerz:

- [Corpus luteum](https://niyora.com/glossary/corpus-luteum/): the structure that forms after ovulation and makes progesterone.

- [Allopregnanolone](https://niyora.com/glossary/allopregnanolone/): a hormone byproduct linked to mood changes in the luteal phase.

- [Menstrual cycle hormones, phase by phase](https://niyora.com/cycle/menstrual-cycle-hormones/): how estrogen and progesterone shift across the month.

Knowing these terms makes it easier to tell a normal cycle sensation from a symptom worth tracking with your doctor. For the fuller picture on what causes PMDD, see [PMDD, explained](https://niyora.com/pmdd/) and [PMS and PMDD research, summarised](https://niyora.com/research/).

## Questions people ask

Is mittelschmerz a sign of PMDD?

+

No. Mittelschmerz happens at ovulation, about two weeks before a period, while PMDD symptoms build in the final week before one. They're different parts of the cycle, driven by different hormone shifts, even though both can feel like pelvic discomfort or mood change.

How long does mittelschmerz last?

+

Mittelschmerz usually lasts anywhere from a few minutes to about one or two days. It tends to feel like a dull ache or a sharp twinge on one side of your lower belly, and it eases on its own without treatment for most people.

Does everyone get mittelschmerz?

+

No. Some people notice mittelschmerz every cycle, some notice it occasionally, and some never feel it at all. Not noticing it doesn't mean you're not ovulating, since plenty of people ovulate without any sensation around it.

Can mittelschmerz happen on both sides in the same cycle?

+

Mittelschmerz usually happens on one side at a time, since it follows the ovary releasing an egg that month. Over several cycles it can switch sides, but feeling it on both sides in one cycle is uncommon.

When should I see a doctor about ovulation pain?

+

Ovulation pain goes beyond typical mittelschmerz if it's severe, lasts more than two or three days, or comes with fever, heavy bleeding, or pain during sex. Those signs call for medical care rather than waiting it out.
