# Luteinizing hormone (LH): what it means

Luteinizing hormone (LH) is a pituitary hormone that surges to trigger ovulation. After ovulation, it helps build the corpus luteum, which makes the progesterone that shapes the luteal phase. Research hasn't tied LH levels themselves to PMS or PMDD, which come from a different reaction to normal hormone shifts, not abnormal hormone levels.

## At a glance

- Luteinizing hormone (LH) is a pituitary hormone that surges to trigger ovulation each cycle.
- After ovulation, LH helps the empty follicle become the corpus luteum, which makes progesterone through the luteal phase.
- LH hasn't been shown to differ between women with and without PMDD, though it's rarely studied on its own, and it isn't a treatment target; what differs is how the brain reacts to normal hormone changes.
- A 2021 meta-analysis of 63 studies found lower luteal estradiol in PMDD (Frontiers in Neuroendocrinology, 2021); it didn't report a separate finding on LH itself.
- LH isn't a treatment target for PMS or PMDD. Treatment instead focuses on how the brain responds to the hormone shifts after ovulation.

## What is luteinizing hormone (LH)?

Luteinizing hormone (LH) is made by the pituitary gland, a small gland at the base of your brain. It's part of the hypothalamic-pituitary-ovarian (HPO) axis, the signaling loop that runs your menstrual cycle.

Across the cycle, LH does two main jobs:

- Triggers the egg's release, with a **sharp surge** about a day before ovulation

- Helps turn the empty follicle into the [corpus luteum](https://niyora.com/glossary/corpus-luteum/), which makes the progesterone that shapes the [luteal phase](https://niyora.com/luteal-phase/)

LH doesn't act alone. It rises and falls alongside estrogen, progesterone and FSH across the cycle. Some ovulation predictor kits test urine for this same LH surge.

## How does LH relate to PMS and PMDD?

On its own, LH isn't what causes PMS or PMDD. Both conditions come from how the brain reacts to the hormone shifts that follow ovulation, not from LH being too high or too low.

Once LH has done its job and ovulation happens, progesterone (made by the corpus luteum) and estrogen rise, then fall sharply in the days before your period. That drop, not LH, is the shift most closely tied to PMS and PMDD.

PMDD is a **different reaction to normal hormone changes**, not an abnormal hormone level. The hormones themselves look much the same as in women without PMDD. What's different is how sensitive the brain is to that normal drop.

If you want the fuller picture of how that plays out day by day, see [PMDD, explained](https://niyora.com/pmdd/).

## What does the research say about LH?

Research directly on LH is limited, and not all of it looks at the days right before a period, but a few related findings stand out.

| Study | What it looked at | What it found |
| --- | --- | --- |
| Frontiers in Neuroendocrinology, 2021 (meta-analysis, 63 studies) | HPO axis hormones in PMDD and depression | Lower luteal estradiol in PMDD; no separate finding on LH itself |
| Journal of the Society for Gynecologic Investigation, 1996 (small trial) | LH response to naloxone in PMS vs. controls | Women with PMS lost the normal midluteal LH rise, a sign of lower opioid activity |
| The Journal of Clinical Endocrinology and Metabolism, 1987 (trial, n=10) | LH pulse pattern during sleep, early follicular phase (not premenstrual) | Pulse frequency and mean LH drop overnight, reversed by naloxone, linked to opioid signaling |

None of this means LH causes premenstrual symptoms. It's one small piece of a larger hormone and brain-chemistry picture researchers are still mapping. See [PMS and PMDD research, summarised](https://niyora.com/research/) for more of it.

## Does LH matter for PMS or PMDD treatment?

LH itself **isn't a treatment target** for PMS or PMDD. Guidelines focus on how the brain responds to the hormone shifts after ovulation, not on raising or lowering LH.

Options a doctor might discuss include:

- SSRIs, which can work even when taken only in the luteal phase

- Certain hormonal contraceptives

- Cognitive behavioral therapy (CBT)

A doctor decides what fits, based on your symptoms and history.

Two related terms worth knowing:

- [Corpus luteum](https://niyora.com/glossary/corpus-luteum/): the structure LH helps form after ovulation

- [Allopregnanolone](https://niyora.com/glossary/allopregnanolone/): a progesterone byproduct some researchers think plays a bigger role in premenstrual mood than LH does

Coping tools can help in the moment, even though they don't treat PMDD. ## Questions people ask

What does luteinizing hormone do in the menstrual cycle?

+

Luteinizing hormone (LH) is a pituitary hormone that surges sharply about a day before ovulation, triggering the egg's release. After ovulation, LH helps the empty follicle become the corpus luteum, which makes the progesterone that shapes the luteal phase and the days before your period.

Does luteinizing hormone cause PMS or PMDD?

+

Luteinizing hormone itself hasn't been shown to cause PMS or PMDD. A 2021 meta-analysis found lower luteal estradiol in PMDD, but didn't report a separate LH finding; PMDD is thought to come from how sensitive the brain is to normal hormone swings, not abnormal hormone levels (Frontiers in Neuroendocrinology, 2021).

What's the difference between LH and FSH?

+

LH and FSH are both pituitary hormones that drive the menstrual cycle together. FSH helps follicles grow earlier in the cycle, while LH's surge triggers ovulation itself and then supports the corpus luteum through the luteal phase.

Is LH tested when diagnosing PMDD?

+

PMDD is diagnosed with daily symptom ratings over at least two cycles, not with an LH blood test. LH levels generally look similar in women with and without PMDD, since PMDD is about the brain's response to hormone shifts, not hormone levels themselves.
