# Estrogen withdrawal: what it means

Estrogen withdrawal is the fall in estrogen that happens in the days right before your period starts, after both estrogen and progesterone have been dropping through the luteal phase. That drop affects brain chemistry tied to mood, which is part of why PMS and PMDD symptoms cluster in this window. The size of the drop isn't unusual in PMDD, it's how strongly the brain reacts to it.

## At a glance

- Estrogen withdrawal is the fall in estrogen (alongside progesterone) in the last days before a period, not a condition on its own.
- It affects serotonin activity in the brain, which helps explain mood changes tied to PMS and PMDD.
- PMDD involves a stronger brain reaction to this normal drop, not an abnormal hormone level.
- Stabilizing hormones (patch or gel estradiol), SSRIs and CBT are options a doctor might discuss, each with different evidence behind it.
- Tracking symptoms against your cycle shows whether estrogen withdrawal fits your pattern.

## What is estrogen withdrawal?

Estrogen withdrawal means the fall in estrogen levels that happens in the final days of the [luteal phase](https://niyora.com/luteal-phase/), just before your period starts. Progesterone falls at the same time, and together these drops set off physical and emotional symptoms many people notice in the week before bleeding, including:

- Mood changes such as irritability, sadness or anxiety

- Physical symptoms like bloating, fatigue or breast tenderness

- Sleep changes, including trouble falling or staying asleep

Estrogen isn't only about reproduction. It shapes serotonin activity in the brain, the same system linked to mood (Biological Psychiatry, 1998). When estrogen falls, that support eases off, which is part of why mood can dip or swing in this window.

**The drop itself is normal.** Everyone who ovulates goes through it every cycle, whether or not they notice symptoms.

## How does estrogen withdrawal relate to PMS and PMDD?

PMS and [PMDD](https://niyora.com/pmdd/) both track the same hormone pattern, but the brain's response differs.

- **PMS**: common physical and mood symptoms (irritability, bloating, low mood) that build in the days before a period and ease once it starts.

- **PMDD**: a diagnosis that needs at least five symptoms, including a mood symptom, confirmed by daily tracking over at least two cycles.

PMDD is a different reaction to a normal hormone drop, not a sign that the hormone levels themselves are unusual (Schmidt et al., 1998, NEJM; Dubey et al., 2017, Molecular Psychiatry). Brain imaging studies also find that estrogen and progesterone act differently on mood-related regions like the amygdala depending on the cycle phase (Psychoneuroendocrinology, 2014). [What causes PMDD](https://niyora.com/questions/what-causes-pmdd/) goes deeper into that difference.

## What does the research say about estrogen withdrawal?

Several lines of research point to estrogen withdrawal as a trigger for premenstrual symptoms.

- A Cochrane review looked at non-contraceptive estrogen patches, gels and implants for PMS, which work by stopping the cyclical rise and fall of hormones that estrogen withdrawal depends on. The evidence was limited and low quality, so it couldn't confirm how well they work (Cochrane, 2017).

- A randomized crossover trial in postmenopausal women found that a higher, continuous estrogen dose combined with a progestin caused more tension, irritability and low mood than a lower estrogen dose with the same progestin, pointing to estrogen dose, rather than estrogen withdrawal, as what worsened mood in that study (Journal of Clinical Endocrinology and Metabolism, 2003).

- An older trial in postmenopausal women given estrogen implants plus a cyclical progestogen reproduced PMS-like symptoms, and researchers have used it as a working model for the condition (British Journal of Obstetrics and Gynaecology, 1986).

This body of research points to the hormone drop as the mechanism most treatments target, even though it doesn't fully explain PMDD on its own.

## What can help with estrogen withdrawal symptoms?

Knowing that a hormone drop, not a character flaw, sits under these symptoms changes what's worth trying.

| Option | What it targets | How it's typically used |
| --- | --- | --- |
| SSRIs | Serotonin's response to hormone shifts | Daily, or only in the luteal phase |
| Hormonal options (patch, gel, some contraceptives) | The estrogen and progesterone swings themselves | Smooths or stops the cyclical drop |
| CBT | Thought and behavior patterns around symptoms | Alongside other treatment |

A doctor decides what fits, usually alongside two months of daily symptom notes.

In the moment, you can't change your hormone levels, but you can change what happens next. Niyora is a PMS and PMDD support app, and its Moon feature lets you write down what happened, see it from another angle and draft a reply you can hold onto for 20 minutes before you send anything.

## Questions people ask

What is estrogen withdrawal?

+

Estrogen withdrawal is the fall in estrogen that happens in the last days before a period, alongside a drop in progesterone. It affects brain chemistry linked to mood, which is part of why PMS and PMDD symptoms often appear in this window. The drop happens every ovulating cycle, whether or not it causes symptoms.

Does estrogen withdrawal cause PMDD?

+

Estrogen withdrawal doesn't cause PMDD on its own. PMDD involves an unusually strong brain reaction to a normal hormone drop, not abnormal hormone levels themselves. The withdrawal is the trigger; the difference lies in how sensitively a person's brain responds to it.

Can treating estrogen withdrawal help PMS or PMDD symptoms?

+

For some people, yes. Research has tested transdermal estrogen, SSRIs and CBT for premenstrual symptoms linked to estrogen withdrawal, with results that vary by person and approach (Cochrane, 2017). A doctor can help decide what fits your pattern and history.

Is estrogen withdrawal the same every cycle?

+

Estrogen withdrawal happens every cycle you ovulate, but how strongly you feel it can shift month to month. Stress, sleep and other factors can change how noticeable symptoms are, even though the underlying hormone pattern stays fairly similar each time.
