# DSM-5 and PMDD: what it means

DSM-5 is the American Psychiatric Association's manual of mental health diagnoses, and it's where PMDD (premenstrual dysphoric disorder) became an official diagnosis in 2013. It lists exactly which symptoms, how many, and over what timing are needed to confirm PMDD, usually tracked daily across two cycles.

## At a glance

- DSM-5 added PMDD as a diagnosis in 2013, under depressive disorders rather than as a hormone condition.
- A PMDD diagnosis needs at least five symptoms in the week before a period, including one mood symptom.
- Symptoms must ease within a few days of the period starting and stay minimal the week after.
- Confirming a diagnosis usually means daily symptom tracking across two or more cycles.
- Researchers built scoring tools like C-PASS to apply the DSM-5 criteria the same way each time.

## What is DSM-5 and PMDD?

DSM-5 is the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders, the reference book clinicians use to diagnose mental health conditions. The current text revision is DSM-5-TR.

Premenstrual dysphoric disorder (PMDD) became an official diagnosis when DSM-5 was published in 2013, listed among the depressive disorders [5].

To meet the DSM-5 criteria, someone needs:

- At least five symptoms in the final week before a period

- At least one of those symptoms is a mood symptom: mood swings, rejection sensitivity, irritability or anger, low mood, or anxiety

- Symptoms that ease within a few days of the period starting, and stay minimal the following week

These patterns need to show up across most cycles, not just once. For what the full diagnosis covers, see [PMDD (premenstrual dysphoric disorder), explained](https://niyora.com/pmdd/).

## How does the DSM-5 definition relate to PMS and PMDD?

PMS and PMDD both show up in the days before a period, but DSM-5 only names one of them.

PMS describes common physical or emotional symptoms in the one to two weeks before bleeding that ease once a period starts. It isn't a DSM-5 diagnosis.

PMDD is the DSM-5 diagnosis, and it needs more: a set number of symptoms, a mood symptom among them, and a pattern confirmed over more than one cycle.

| | PMS | PMDD |
| --- | --- | --- |
| In DSM-5? | No | Yes, since 2013 |
| Symptom count | Varies | At least five |
| Mood symptom required | No | Yes |
| How it's confirmed | Self-reported pattern | Daily ratings over two or more cycles |

What's happening hormonally in the days before a period is the same for both: estrogen and progesterone drop in the luteal phase. Read more about [the luteal phase: what's happening in your body and mood](https://niyora.com/luteal-phase/).

When to see a doctor

If this pattern takes a week of every month, or shows up most cycles, bring two months of daily notes to a GP or gynecologist. A record like that matters more than memory for a DSM-5 diagnosis.

## What does the research say about diagnosing it this way?

Applying diagnostic criteria for PMDD consistently has always been harder than it sounds. Even under DSM-IV, the manual that came before DSM-5, researchers ran into this.

One scoring system, the Carolina Premenstrual Assessment Scoring System (C-PASS), works through this by scoring two or more months of daily ratings against the DSM-5 criteria [1].

The numbers show why the method matters so much:

- A literal yes or no threshold for symptoms correctly identified only 28% of symptomatic cycles, compared with 92% using the study's optimal combination of thresholds (Journal of Psychiatric Research, 2003) [2]

- An earlier study found PMDD prevalence estimates ranging from 1.0% to 7.1% depending on how DSM-IV symptoms were measured (Journal of Psychosomatic Obstetrics and Gynaecology, 1997) [3]

DSM-5 built on this work, and C-PASS applies the current criteria the same way each time.

Screening questionnaires built on DSM-5 criteria have since been validated in Spanish and Chinese samples, which helps the diagnosis travel across languages [4] [6].

One review cautioned that clinicians should be wary about using the newer DSM-5 term as a diagnosis for patients (Tijdschrift voor psychiatrie, 2014) [5].

Daily tracking, not memory, is what carries the diagnosis.

What the research says

A 2017 study using C-PASS matched expert clinical diagnoses in 98% of cases when it scored daily symptom ratings against the DSM-5 criteria (American Journal of Psychiatry, 2017).

## Why does this matter, and what related terms should you know?

Getting the DSM-5 criteria right has real consequences. It can decide who qualifies for treatment guided by a PMDD diagnosis: SSRIs taken only in the luteal phase, certain hormonal contraceptives, or CBT, with a doctor deciding what fits.

A few related terms worth knowing:

- Corpus luteum: the ovarian structure that makes the progesterone and estrogen that fall before a period. See [Corpus Luteum: what it means](https://niyora.com/glossary/corpus-luteum/).

- Allopregnanolone: a hormone byproduct some brains react to differently in PMDD. See [Allopregnanolone: what it means](https://niyora.com/glossary/allopregnanolone/).

In the moment

If you want to see patterns over two cycles instead of relying on memory, Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what's happening in the days before your period as it happens, which can help you notice a pattern across a few cycles, though it isn't the structured daily symptom rating a DSM-5 diagnosis requires.

## Questions people ask

Is PMDD in the DSM-5?

+

Yes. Premenstrual dysphoric disorder was added to DSM-5 in 2013, listed among the depressive disorders. DSM-5 sets out specific criteria for PMDD: at least five symptoms, one of them a mood symptom, confined to the week before a period and easing within days of it starting.

How many symptoms does DSM-5 require for a PMDD diagnosis?

+

DSM-5 requires at least five symptoms in the week before a period, with at least one being a mood symptom: mood swings, irritability or anger, low mood, or anxiety. Symptoms need to ease within days of the period starting and stay minimal the following week.

Can a doctor diagnose PMDD from one month?

+

Not reliably. A DSM-5 PMDD diagnosis depends on tracking symptoms daily across at least two menstrual cycles, since retrospective recall is a poor predictor of diagnosis (American Journal of Psychiatry, 2017). Tools like C-PASS apply this consistently.

Is PMS also a DSM-5 diagnosis?

+

No. PMS describes common physical or emotional symptoms before a period that ease once it starts, but it isn't a DSM-5 diagnosis. PMDD is the DSM-5 diagnosis, with specific symptom counts, a required mood symptom, and confirmation through daily tracking over more than one cycle.

Does a DSM-5 PMDD diagnosis change treatment options?

+

A confirmed PMDD diagnosis can guide treatment choices, including SSRIs taken only in the luteal phase, some hormonal contraceptives, or CBT, with a doctor deciding what fits. Bring two months of daily notes to a GP or gynecologist if this takes a week of every month.
