# Cortisol and PMS: what it means

Cortisol is the hormone your adrenal glands release in response to stress, and it also rises and falls on its own daily rhythm. It shifts slightly across your menstrual cycle, running a bit higher before ovulation than after. In PMDD, it's the timing of cortisol's daily peak that shifts in the week before your period, not the overall amount.

## At a glance

- Cortisol is the main stress hormone, but it also follows its own daily rhythm, independent of your cycle.
- Cortisol runs slightly higher in the follicular phase than the luteal phase in healthy women (Hedges' g 0.13) [1].
- In PMDD, it's the timing of cortisol's daily peak that shifts in the late luteal phase, not necessarily the amount [2].
- The HPA axis, which controls cortisol, works the same way in women with and without PMDD once hormones are controlled [5].
- Steady sleep timing may help cortisol's rhythm stay regular; this is a coping idea, not a PMDD treatment.

## What is cortisol?

Cortisol is the hormone your adrenal glands release when your body senses stress. It also follows its own daily rhythm, rising before you wake and falling through the evening, whether or not anything stressful is happening.

It does a few concrete jobs:

- Raises blood sugar so your muscles and brain get fast fuel

- Sharpens short-term focus and alertness

- Dials back non-urgent functions like digestion while the stress response is active

**Cortisol isn't inherently bad.** The trouble comes from how high it runs and for how long, not from its existence. Your levels also shift across the menstrual cycle on their own, which is where cortisol and PMS start to overlap. More on that shift in the [luteal phase](https://niyora.com/luteal-phase/).

Where this sits in your cycle

Luteal phase Days 15 to 28

Day 1

Ovulation

Day 28

## How does cortisol relate to PMS and PMDD?

Cortisol shifts with your cycle, not only with stress. A meta-analysis of 778 women found cortisol runs slightly higher in the follicular phase (before ovulation) than in the luteal phase (Hedges' g 0.13, p < 0.01) [1].

In PMDD, the difference isn't how much cortisol you make:

- A study of 15 women with PMDD and 15 without found similar overall cortisol amounts in both groups [2]

- What differed was timing: moving into the late luteal phase, women without PMDD showed a cortisol peak about an hour earlier than usual, a shift that didn't happen in women with PMDD. After an earlier night's sleep, women with PMDD showed a peak almost two hours earlier than usual [2]

- A separate lab study found a stress test raised cortisol more during the follicular phase than the luteal phase, though its effect on memory recall was about the same either way [4]

- When researchers controlled estrogen and progesterone directly, the system that produces cortisol (the HPA axis) worked the same way in women with and without PMDD [5]

Taken together, this points to PMDD as a different brain reaction to the normal hormone drop before your period, not a cortisol problem on its own. Read more about [what causes PMDD](https://niyora.com/questions/what-causes-pmdd/).

## What can help with cortisol and PMS stress?

Nothing lowers cortisol as a treatment for PMS or PMDD, but a few things can take the edge off in the moment and steady the daily rhythm over time.

In the moment:

- Name what's happening: "this is a stress spike, and it will pass"

- Breathe out for six counts, three times, to settle the alertness cortisol creates

- Write down what you're reacting to before you respond to anyone

| Approach | What it does | Evidence |
| --- | --- | --- |
| Steady sleep timing | Keeps the daily cortisol rhythm more regular, which may matter more in PMDD | Small study, limited [2] |
| Magnesium | Modest, inconsistent anxiety relief in some PMS samples | Mixed, limited [3] [6] |
| SSRIs, some hormonal contraceptives, CBT | Target the mood symptoms a hormone shift can worsen; a doctor decides what fits | Guideline-recommended |

If stress spikes faster than you can think straight, that's worth having a plan for. Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what happened, see it from another angle, and draft a reply you can hold onto for 20 minutes instead of sending right away.

## Related terms to know

A few terms show up alongside cortisol when people search this topic:

- [Corpus luteum](https://niyora.com/glossary/corpus-luteum/): the structure that makes progesterone after ovulation; its breakdown triggers the hormone drop linked to PMS

- [Allopregnanolone](https://niyora.com/glossary/allopregnanolone/): a calming neurosteroid made from progesterone, often discussed alongside cortisol's daily rhythm

None of this replaces a diagnosis. If you want the full picture of symptoms, causes and diagnosis, see [PMDD, explained](https://niyora.com/pmdd/).

When to see a doctor

If stress or mood symptoms take a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## Questions people ask

What is cortisol, in simple terms?

+

Cortisol is the hormone your adrenal glands release in response to stress, and it also rises and falls on its own daily rhythm. It helps your body access energy quickly and dials back less urgent functions like digestion. Cortisol interacts with the hormone shifts of the luteal phase, but it isn't the same thing as PMS or PMDD.

Does cortisol cause PMS or PMDD?

+

Cortisol doesn't cause PMS or PMDD. A study of 15 women with PMDD and 15 without found similar overall cortisol levels; what differed was the timing of cortisol's daily peak in the late luteal phase [2]. PMDD looks like a different brain reaction to normal hormone shifts, not a cortisol problem on its own.

Is cortisol higher before your period?

+

Cortisol tends to run slightly lower in the luteal phase (after ovulation) than in the follicular phase, according to a meta-analysis of 778 women (Hedges' g 0.13) [1]. The difference is small on average, and it doesn't predict what any one person will feel in the days before her period.

Can lowering cortisol help with PMS symptoms?

+

There's no study showing that lowering cortisol specifically treats PMS or PMDD. Steady sleep timing may help keep cortisol's daily rhythm regular, and magnesium has shown mixed, limited results for anxiety in some PMS samples [3] [6]. A doctor can advise on SSRIs or CBT, which guidelines recommend for PMDD's mood symptoms.

What's the difference between cortisol and allopregnanolone?

+

Cortisol is a stress hormone with its own daily rhythm. Allopregnanolone is a calming neurosteroid made from progesterone, and it falls sharply before your period. Some researchers think the two move in different directions across the cycle, though this needs more direct study to confirm.
