# Sadness in the luteal phase: why it happens

Sadness in the luteal phase is common and usually tied to the drop in progesterone, estrogen and progesterone's byproduct allopregnanolone in the days before your period. For most women it builds in the final week and lifts within days of bleeding starting. If it takes over a week every month, that pattern is worth tracking and mentioning to a doctor.

Days 15 to 28

Where this sits in your cycle - Period - Building - Ovulation - Luteal - PMS window

## At a glance

- Progesterone and estrogen fall sharply in the final days of the luteal phase, and progesterone's byproduct allopregnanolone affects how calm your brain feels.
- Sadness that fits a PMDD pattern tends to build in the late luteal phase and ease within days of your period starting.
- PMS is common and manageable. PMDD is a diagnosis confirmed by daily symptom ratings over at least two cycles.
- Naming the feeling, getting outside and lowering tonight's expectations can help in the moment.
- Tracking mood daily for two cycles turns a vague bad feeling into a pattern you can see coming.

## Strength of evidence

Moderate evidence

- Limited
- Mixed
- Moderate
- Strong

Links between falling progesterone, allopregnanolone and luteal-phase mood come from small trials and cohort studies with consistent direction but limited sample sizes. Treatment evidence for luteal-only SSRI dosing and some contraceptives is stronger but still from modest trials.

## Why do you feel sad in the luteal phase?

Your luteal phase starts right after ovulation and ends when your period begins (see [what's happening in your body during this phase](https://niyora.com/luteal-phase/)). Three things shift together in the final days before bleeding:

- Progesterone, which rises earlier in the phase then drops sharply

- Estrogen, which falls alongside it

- Allopregnanolone, progesterone's calming byproduct, which falls as progesterone does

Progesterone breaks down into allopregnanolone, a hormone that normally calms activity in the brain. In a trial that blocked this conversion with a drug, women with PMDD had fewer luteal-phase symptoms of sadness and irritability at the higher dose, which points to allopregnanolone's role in how the brain reacts to the cycle (2016) [2].

Falling progesterone across the luteal phase, not just how low it gets, predicted the severity of premenstrual symptoms including low mood in a study that tracked 41 women daily for a full cycle (2019) [4]. **The drop itself seems to matter as much as the hormone level.**

## Is sadness in the luteal phase normal?

Yes. Mild sadness, tearfulness or a flatter mood in the days before your period is one of the most common parts of the luteal phase, and it usually eases once bleeding starts.

Not everyone reacts to the same hormone shift the same way. In a study comparing women with and without PMDD, only the PMDD group showed a clear rise in dysphoric mood from the follicular phase to the late luteal phase, a rise that was significantly larger than in the comparison group (2008) [5]. For more on this pattern, see mood in the luteal phase.

So the sadness you feel before your period might be a hormone shift most women notice quietly, or a stronger reaction to that same shift. If crying is more your experience than sadness, see [sadness before your period](https://niyora.com/symptoms/sadness-before-period/). Either way, what you're feeling is real.

Right now

If sadness is heavy right now

- 1 Name it: say or write that this is luteal phase sadness and it usually passes in a few days.
- 2 Step outside for ten minutes. Light and movement both shift mood on their own.
- 3 Text one person who doesn't need the backstory, just company.
- 4 Write down what you're feeling before you act on it or send a message you can't take back.
- 5 Let tonight's to-do list wait until tomorrow.

- Calm down

## When in the luteal phase is it worst?

For most women, sadness builds in the final days before the period, a window often called the late luteal phase.

Women with PMDD show a stronger startle response than women without PMDD across the whole cycle. But their ability to filter out repeated sounds, a measure of how well the brain calms itself, drops specifically in the late luteal phase (2008) [3]. That's one sign the brain's usual calming systems work least well right before your period.

Appetite and memory shift in the same window. In one study, women with PMDD ate more high-fat food and did worse on memory tasks during the late luteal phase, compared to their own follicular-phase scores and to women without PMDD (2008) [5].

**The last stretch before your period, often called the late luteal phase, is usually when sadness is heaviest**, and it should start lifting within a few days of bleeding starting. See the full list of [luteal phase symptoms](https://niyora.com/cycle/luteal-phase-symptoms/) to check what else tends to cluster in this window.

## Is it PMS or PMDD?

PMS and PMDD sit on the same hormonal timeline, but they differ in how much they take over your life.

PMS means physical or emotional symptoms in the one to two weeks before your period that ease once it starts. It's common and usually workable. PMDD is a diagnosis in the DSM-5-TR and ICD-11, confirmed by daily symptom ratings over at least two cycles.

| | PMS | PMDD |
| --- | --- | --- |
| Symptom count | One or more, physical or emotional | At least five, including one mood symptom |
| Timing | One to two weeks before period | Mostly the final week before period |
| How it's confirmed | Usually by pattern alone | Daily ratings over at least two cycles |
| Effect on daily life | Noticeable but workable | Disrupts work, relationships or both |

Some hormonal contraceptives ease premenstrual mood symptoms, and an SSRI taken only in the luteal phase can also help, though a doctor decides what fits your situation. An intermittent dosing approach worked better than placebo for luteal-phase irritability in one trial (1998) [6].

When to see a doctor

If this takes a week of every month, or you have thoughts of hurting yourself, see a doctor or gynecologist and bring two months of daily notes.

## What helps right now?

When sadness is heavy in the evening, you don't need to fix the whole cycle. You need something small that works tonight.

- Name it out loud or in writing, something like "this is luteal phase sadness, it usually eases in a few days."

- Get outside for ten minutes if you can. Light and movement both affect mood on their own.

- Text one person who doesn't need an explanation, just company.

- Lower the bar for tonight. Dishes and email can wait until tomorrow.

If you want somewhere to put the feeling before you decide what to do with it, Niyora is a PMS and PMDD support app. Its Moon feature lets you write down what happened, see it from another angle and calm down before you respond to anyone.

## How do you track it across cycles?

Tracking turns a vague sense of dread into a pattern you can actually see coming.

- Pick a simple 1-to-5 mood scale and rate it once a day, around the same time if you can.

- Note the cycle day relative to your period, not the calendar date.

- Mark when sadness starts and when it lifts after bleeding begins.

- Keep this up for at least two cycles before you look for a pattern.

Doctors use a similar daily rating approach, called the Daily Record of Severity of Problems, to tell PMS and PMDD apart from conditions that simply get worse before the period without easing afterward (2019) [4]. If your notes show sadness isn't tied to your cycle at all, that's useful information too, not a failed attempt at tracking.

## Questions people ask

Does the luteal phase always make you sad?

+

Luteal phase sadness is common but not universal. Falling progesterone and estrogen before your period affect mood in many women, and some brains react more strongly to that drop than others. If sadness shows up most months and eases within days of your period starting, it's likely tied to your cycle.

Why is luteal phase sadness worse some months than others?

+

Luteal phase sadness can vary month to month because life stress, sleep and the size of the hormone swing all interact with your cycle. Hormone levels that predict symptoms best are the average level and how far it falls, not a single fixed number, according to a 2019 study that tracked progesterone levels in 41 women daily across a full cycle [4].

Can birth control help with luteal phase sadness?

+

Some hormonal contraceptives ease premenstrual mood symptoms, though effects vary by person. In one trial, combined oral contraceptives improved depression scores in the premenstrual phase but slightly increased anxiety and irritability between periods (2017). A doctor can help you weigh this against your own pattern.

Is luteal phase sadness the same as depression?

+

Luteal phase sadness that lifts within days of your period starting differs from ongoing depression, which continues through the whole cycle. Premenstrual exacerbation describes an existing condition, like depression, that gets worse before the period but doesn't go away afterward. Daily mood tracking for two cycles helps tell these apart.

When should you see a doctor about luteal phase sadness?

+

Consider seeing a doctor about luteal phase sadness if it takes over a week of most months or disrupts work and relationships. Bring two months of daily mood notes, along with when they happen relative to your period. That pattern is what helps diagnose PMDD, not a single day.
